Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliances. The following information describes vaccine compliance rules as defined by the Oregon State Department of Health.

DTaP

Rule	Requirements
5 Dose Complete	Students are considered complete when: the student is in state grade(s) KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, or 12, and the student receives 5 doses of the DTaP-DTP, DT, Td, or Tdap vaccine.
4 Dose Complete	Students are considered complete when: the student receives 4 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and the difference between the last dose and the student's birthdate is greater than or equal to 48 months with a 4-day tolerance.
3 Dose Complete	Students are considered complete when: the student receives 3 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and the difference between the last dose and the student's birthdate is greater than or equal to 84 months with a 4-day tolerance.
4 Dose Up-To-Date	Students are considered up-to-date when: the student is in state grade(s) PK, the student receives 4 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and the difference between the last dose and the student's birthdate is less than or equal to 48 months with a 4 day tolerance.
3 Dose Up-To-Date	Students are considered up-to-date when: the student receives 3 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and there is a dose in the last 12 months with a 0-day tolerance.
3 Dose < 18mos Up-To-Date	Students are considered up-to-date when: the student is no older than 17 months old, and the student receives 3 doses of the DTaP-DTP, DT, Td, or Tdap vaccine.
3 Dose < 4yrs Up-To-Date	Students are considered up-to-date when: the student is no older than 47 months old, the student receives 3 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and the difference between the first dose and the student's birthdate is greater than or equal to 12 months with a 4 day tolerance.
2 Dose Up-To-Date	Students are considered up-to-date when: the student receives 2 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 12 months, and there is a dose in the last 12 months with a 0-day tolerance.
2 Dose > 7yrs Up-To-Date	Students are considered up-to-date when: the student is at least 84 months old, the student receives 2 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and there is a dose in the last 12 months with a 0-day tolerance.
2 Dose < 6mos Up-To-Date	Students are considered up-to-date when: the student is no older than 5 months old, and the student receives 2 doses of the DTaP-DTP, DT, Td, or Tdap vaccine.
2 Dose (no 1st dose rule) Up-To-Date	Students are considered up-to-date when: the student receives 2 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and there is a dose in the last 2 months.
1 Dose Up-To-Date	Students are considered up-to-date when: the student receives 2 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and there is a dose in the last 2 months.
1 Dose < 4mos Up-To-Date	Students are considered up-to-date when: the student is no older than 3 months old, and the student receives 1 dose of the DTaP-DTP, DT, Td, or Tdap vaccine.

Polio

Rule	Requirements
Over 18yrs No Requirement	Students 216 months old and older are not required to receive any doses of the Polio vaccine.
4 Dose Complete	Students are considered complete when: the student is in state grades KG - 12, UM, or US, the student receives 4 doses of Polio (IPV/OPV) or IPV, and the difference between the third dose and the effective date is greater than or equal to 12 months with a 1 day tolerance.
3 Dose Complete	Students are considered complete when: the student receives 3 doses of Polio (IPV/OPV) or IPV, and the difference between the last dose and the student's birthdate is greater than or equal to 48 months with a 4 day tolerance.
3 Dose Up-To-Date	Students are considered up-to-date when: the student receives 3 doses of Polio (IPV/OPV) or IPV, and there is a dose in the last 12 months with a 0-day tolerance.
3 Dose Pre K Up-To-Date	Students are considered up-to-date when: the student is in state grade(s) PK, the student receives 3 doses of Polio (IPV/OPV) or IPV, and there is a dose in the last 12 months.
2 Dose Up-To-Date	Students are considered up-to-date when: the student receives 2 doses of Polio (IPV/OPV) or IPV, and there is a dose in the last 12 months with a 0-day tolerance.
2 Dose < 18mos Up-To-Date	Students are considered up-to-date when: the student is no older than 17 months old, and the student receives 2 doses of Polio (IPV/OPV) or IPV.
1 Dose Up-To-Date	Students are considered up-to-date when: the student receives 1 dose of the Polio (IPV/OPV) or IPV, and there is a dose in the last 2 months.

Rule	Requirements
Age <18 Mos No Requirement	Students 17 months old and younger are not required to receive any doses of the Varicella vaccine.
1 Dose (1 dose btw 12-156mos) Complete	Students are considered complete when: the student receives 1 dose of the Varicella or MMRV vaccine, the difference between the last dose and the student's birthdate is greater than or equal to 12 months with a 4 day tolerance, and the difference between the last dose and the student's birthdate is less than or equal to 155 months with a 1 day tolerance.
2 Dose (1st dose after 13yrs) Complete	Students are considered complete when: the student receives 2 doses of the Varicella or MMRV vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 156 months with a 4 day tolerance, and the difference between the second and previous dose is greater than or equal to 28 days with a 4 day tolerance.
3 Dose Complete	Students are considered complete when the student receives 3 doses of the Varicella or MMRV vaccine.
1 Dose Up-To-Date	Students are considered up-to-date when: the student receives 1 dose of the Varicella or MMRV vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 156 months with a 4 day tolerance, and there is a dose in the last 2 months.

Measles Mumps Rubella

Rule	Requirements
Age < 15mos No Requirement	Students 14 months old or younger are not required to receive any doses of the MMR vaccine.
1 Dose PK Complete	Students are considered complete when: the student is in state grade(s) PK, the student receives 1 dose of the MMR or MMRV vaccine, and the difference between the last dose and the student's birthdate is greater than 12 months with a 4-day tolerance.
1M, 1M, 1R - PK Complete	Students are considered complete when: the student is in state grade(s) PK, the student receives one dose of the Measles vaccine, and the difference between the last dose and the student's birthdate is greater than 12 months with a 4-day tolerance, OR the student receives 1 dose of the Mumps vaccine, and the difference between the last dose and the student's birthdate is greater than 12 months with a 4-day tolerance, OR the student receives 1 dose of the Rubella vaccine, and the difference between the last dose and the student's birthdate is greater than 12 months with a 4-day tolerance.
3 Doses Complete	Students are considered complete when: the student is in state grade(s) K-12, UM, UE, or US, the student receives three doses of the MMR or MMRV vaccine.
3M, 1M, 1R Complete	Students are considered complete when: the student is in state grade(s) K-12, UM, UE, or US, the student receives 3 doses of the Measles vaccine, the student receives 1 dose of the Mumps vaccine, and the student receives 1 dose of the Rubella vaccine.
2MMR, 1M Complete	Students are considered complete when: the student is in state grade(s) K-12, UM, UE, or US, the student receives 2 doses of the MMR or MMRV vaccines, and the student receives 1 dose of the Measles vaccine.
1MMR, 2M Complete	Students are considered complete when: the student is in state grade(s) K-12, UM, UE, or US, the student receives 1 dose of the MMR or MMRV vaccine, and the student receives 2 doses of the Measles vaccine.
2 Doses Complete	Students are considered complete when: the student is in state grade(s) K-12, UM, UE, or US, the student receives 2 doses of the MMR or MMRV vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 12 months with a 4 day tolerance, and the difference between the second and previous dose is greater than or equal to 28 days with a 4 day tolerance.

2 Dose > 11yrs Complete	Students are considered complete when: the student receives 2 doses of the Hepatitis B vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 132 months with a 4 day tolerance, and the difference between the second and previous dose is greater than or equal to 4 months with a 1 day tolerance.
2 Dose > 18yrs Complete	Students are considered complete when: the student receives 2 doses of the Hepatitis B vaccine, and the difference between the first dose and the student's birthdate greater than or equal to 216 months with a 4 day tolerance.
2 Dose Up-To-Date	Students are considered up-to-date when: the student receives 2 doses of the Hepatitis B vaccine, and there is a dose in the last 5 months.
2 Dose < 18mos Up-To-Date	Students are considered up-to-date when: the student is no older than 17 months, the student receives 2 doses of the Hepatitis B vaccine, and there is a dose in the last 5 months.
1 Dose >11yrs Up-To-Date	Students are considered up-to-date when: the student receives 1 dose of the Hepatitis B vaccine, the difference between first dose and the student's birthdate is greater than or equal to 132 months with a 4 day tolerance, and there is a dose in the last 6 months.

4 Dose Complete	Students are considered complete when they receive 4 doses of the Hib vaccine.
3 Dose Complete	Students are considered complete when: the student is at least 12 months, but no older than 59 months old, the student receives 3 doses of the Hib vaccine, and the difference between the last dose and the student's birthdate is greater than or equal to 12 months with a 4 day tolerance.
1 Dose Complete	Students are considered complete when: the student is no older than 59 months old, the student receives 1 dose of the Hib vaccine, and the difference between the last dose and the student's birthdate is greater than or equal to 15 months with a 4 day tolerance.
2 Dose (2-17mos) Up-To-Date	Students are considered up-to-date when: the student is at least 2 months, but no older than 17 months old, the student receives 2 doses of the Hib vaccine, and the difference between the first dose and the student's birthdate is less than or equal to 12 months with a 4 day tolerance.
