The following information describes vaccine compliance rules as defined by the Oregon State Department of Health.

DTaP

5 Dose
Complete

Students are considered complete when:

  • the student is in state grade(s) KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, or 12, and
  • the student receives 5 doses of the DTaP-DTP, DT, Td, or Tdap vaccine.
4 Dose
Complete

Students are considered complete when: 

  • the student receives 4 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and
  • the difference between the last dose and the student's birthdate is greater than or equal to 48 months with a 4-day tolerance.
3 Dose
Complete

Students are considered complete when: 

  • the student receives 3 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and
  • the difference between the last dose and the student's birthdate is greater than or equal to 84 months with a 4-day tolerance.
4 Dose
Up-To-Date

Students are considered up-to-date when: 

  • the student is in state grade(s) PK,
  • the student receives 4 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and
  • the difference between the last dose and the student's birthdate is less than or equal to 48 months with a 4 day tolerance.
3 Dose
Up-To-Date

Students are considered up-to-date when: 

  • the student receives 3 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and
  • there is a dose in the last 12 months with a 0-day tolerance.
3 Dose < 18mos
Up-To-Date

Students are considered up-to-date when: 

  • the student is no older than 17 months old, and
  • the student receives 3 doses of the DTaP-DTP, DT, Td, or Tdap vaccine.
3 Dose < 4yrs
Up-To-Date

Students are considered up-to-date when:

  • the student is no older than 47 months old, 
  • the student receives 3 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and
  • the difference between the first dose and the student's birthdate is greater than or equal to 12 months with a 4 day tolerance. 
2 Dose
Up-To-Date

Students are considered up-to-date when:

  • the student receives 2 doses of the DTaP-DTP, DT, Td, or Tdap vaccine,
  • the difference between the first dose and the student's birthdate is greater than or equal to 12 months, and
  • there is a dose in the last 12 months with a 0-day tolerance.
2 Dose > 7yrs
Up-To-Date

Students are considered up-to-date when:

  • the student is at least 84 months old, 
  • the student receives 2 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and
  • there is a dose in the last 12 months with a 0-day tolerance.
2 Dose < 6mos
Up-To-Date

Students are considered up-to-date when:

  • the student is no older than 5 months old, and
  • the student receives 2 doses of the DTaP-DTP, DT, Td, or Tdap vaccine.

Students are considered up-to-date when:

  • the student receives 2 doses of the DTaP-DTP, DT, Td, or Tdap vaccine, and
  • there is a dose in the last 2 months.
