The Student Health Immunization Report provides a grade by grade breakdown of immunization compliance of all students within a district.

Selection Criteria
Students must meet the following criteria in order to report on the Student Health Immunization Report:
- Calculations are made using data from all students within a district, with totals counted from the current school year (as specified by Effective Date entered on the report editor).
- If the Effective Date is after a school year is over, the most recent school year is used for calculating and reporting student data.
- Calendars marked as State Exclude, grades marked as Exclude from State Reporting and students marked Exclude are not included in the report.
Generate the Report
- Enter the Effective Date. If this date does not fall within an active school calendar date, the most recent school year is used.
- Select Generate Report. The report will appear in a separate window in PDF format.
