Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliances. The following information describes vaccine compliance rules as defined by the Kentucky Department of Health and Family Services.

General Compliance

  • One month is equal to 28 days.
  • One year is equal to the person's birth date, minus 4 days.
  • Six months is equal to 28 days x 6 - 4 (168 - 4 = 164 days).
  • Two doses of Measles containing vaccine required for students born Oct 1, 1992 or later (first does must be MMR).
  • Hep B is required for all students born Oct 1, 1992 or later. Students born before October 1, 1992 are exempt.
  • Born dates are 12 years of age as of January 1st of the current year for the Hepatitis B validation. 

Immunizations

The following tables describe all Kentucky immunization requirements per vaccine.

DTap

1 Dose
Compliant
Students are considered compliant when the following conditions have been met:
  • the student is at least 38 days old, but no older than 3 months of age, and
  • the student receives one dose of the DTaP, DTP, DT or Td vaccine.
2 Doses
Compliant
Students are considered compliant when the following conditions have been met:
  • the student is at least 66 days old,
  • the student receives a second dose of the DTaP, DTP, DT or Td vaccine, and
  • the student receives the second dose 24 days after the first dose.
3 Doses
Compliant
Students are considered compliant when the following conditions have been met:
  • the student is at least 94 days old, 
  • the student receives a third dose of the DTaP, DTP, DT or Td vaccine, and
  • the student receives the third dose 24 days after the second dose.
4 Doses
Compliant
Students are considered compliant when the following conditions have been met:
  • the student is at least 361 days old, 
  • the student receives a fourth dose of the DTaP, DTP, DT or Td vaccine, and
  • the student receives the fourth dose no less than 108 days (4 months) after the third dose.
5 Doses
Compliant
Students are considered compliant when the following conditions have been met:
  • the student is at least 361 days old, 
  • the student receives a fourth dose of the DTaP, DTP, DT or Td vaccine, and
  • the student receives the fourth dose no less than 164 days after the fourth dose.

A fifth dose is not necessary when the fourth dose is given after 3 years and 361 days of age (before the student's fourth birthday).

3 Doses, 1 Tdap - age 7+
Compliant
Students are considered compliant when the following conditions have been met:
  • the student is age 7 or older,
  • the student receives three doses of the DTaP, DTP, DT, or Td vaccine, and
  • at least one of those doses is Td or Tdap.

Tdap or Td

For students entering 6th grade as of July 1st of current year:

Conditions

1 Dose

For the 2014-2015 calendar year, when the student is in grades 7-9 and has not received a Tdap dose, the student is considered Non-Compliant.
  • When the student is in grades 7-9 and they have had a Tdap dose, they display as No-Requirement.
For the 2015-2016 calendar year, when the student is in grades 7-10 and has not received a Tdap dose, the student is considered Non-Compliant.
  • When the student is in grades 7-10 and they have had a Tdap dose, they display as No-Requirement.
For the 2016-2017 calendar year, when the student is in grades 7-11 and has not received a Tdap dose, the student is considered Non-Compliant
  • When the student is in grades 7-11 and they have had a Tdap dose, they display as No-Requirement.
For the 2017-2018 calendar year, when the student is in grades 7-12 and has not received a Tdap dose, the student is considered Non-Compliant
  • When the student is in grades 7-12 and they have had a Tdap dose, they display as No-Requirement.

Hepatitis B

Adolescents age 11-15 years may receive 2 doses of adult Hep B vaccine, no less than 108 days apart. This would be considered a complete series.

3 doses of Pediatric Hep B or 2 doses of Adult Hep B are accepted for compliance.

1 dose

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is 3 months old, 
  • the student receives one dose of the Hepatitis B vaccine.

2 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is 5 months old, 
  • the student receives the second dose of the Hepatitis B vaccine at least 24 days after the first dose.

3 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 164 days (19 months) old, 
  • the student receives the third dose of the Hepatitis B vaccine at least 108 days after the first dose.

MMR

When Varicella and MMR are not administered at the same time, the second live vaccine (whichever is given second) should be repeated after 28 days. The four day grade period does not apply in this instance.

0-15 months

No requirement

Students age 0-15 months old are not required to receive any dose of the MMR vaccine.

1 dose

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 361 days old, and
  • the student receives one dose of the MMR vaccine.

2 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 389 days old, and
  • the student receives the second dose of the MMR or Measles containing vaccine at least 24 days after the first dose

Varicella

When Varicella and MMR are not administered at the same time, the second live vaccine (whichever is given second) should be repeated after 28 days. The four day grade period does not apply in this instance.

Rules

0-16 months

No requirement

Students age 0-16 months old are not required to receive any dose of the Varicella vaccine.

1 dose

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is no older than 48 months old,
  • the student receives one dose of the Varicella vaccine,
  • the first dose is received on or after the student's first birthday with a 4 day tolerance.

2 doses

Compliant

A second dose is only required for those students the meet the following rules based on grade and calendar year:

  • 2014-2015 - When the student is in grade Pre-K, K, 1, 2, 6, 7, 8 or 9, 2 doses are required. All other students only require 1 dose. 
  • 2015-2016 - When the student is in grade Pre-K, K, 1, 2, 3, 4, 6, 7, 8, 9 or 10, 2 doses are required. All other students only require 1 dose.
  • 2016-2017 - When student is in grade Pre-K, K, 1, 2, 3, 4, 5, 6, 7, 8, 9, 10 or 11, 2 doses are required. All other students only require 1 dose.

The student must be at least 48 months old for the above to apply.


Students are considered compliant when the following conditions have been met:

  • the student is at least 389 days old,
  • the student receives the second dose of the Varicella vaccine at least 24 days after the first dose.

Polio

No requirement

Students age 0-2 months old are not required to receive any dose of the Polio vaccine.

1 dose

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 38 days old, 
  • the student receives one dose of the Polio IPV/OPV vaccine.

2 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 66 days old,
  • the student receives the second dose of the Polio IPV/OPV vaccine at least 24 days after the first dose.

3 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 94 days old, but no older than 47 months old,
  • the student receives the third dose of the Polio IPV/OPV vaccine at least 24 days after the second dose.
4 doses
Compliant

When 3 doses of IPV were given before 3 years plus 361 days of age, a fourth booster shot must be administered.


Students are considered compliant when the following conditions have been met:

  • the student is at least 122 days old,
  • the student receives the fourth dose of the Polio IPV/OPV vaccine at least 24 days after the third dose.

Pneumococcal

RuleRequirements

0-2 months

No requirement

Students age 0-2 months old are not required to receive any dose of the Pneumococcal vaccine.

1 dose

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 38 days old (3-4 months),
  • the student receives one dose of the Pneumococcal (PCV) vaccine.

2 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 66 days old (5-6 months),
  • the student receives the second dose of the Pneumococcal (PCV) vaccine at least 24 days after the first dose, UNLESS the second dose is the last dose for the student, then the difference is at least 52 days between the second and first dose.

When second dose is administered on or after the student is 7 months old, they only need two doses.


This dose is required when the first dose was administered on or after the student's first birthday (12 months). 

3 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 94 days old (7-11 months),
  • the student receives the third dose of the Pneumococcal (PCV) vaccine at least 24 days after the second dose, UNLESS the second dose is the last dose for the student, then the difference is at least 52 days between the second and first dose.

This dose is required when the first dose was administered on or after the student's is 7 months old and the last dose was administered on or after the student's first birthday (12 months).

4 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 361 days old (12 months-4 years),
  • the student receives the fourth dose of the Pneumococcal (PCV) vaccine at least 52 days after the second dose.

This dose is required when the last dose was administered on or after the student's first birthday (12 months).

Hib Haemophilus influenzae type B

  • When a dose of HIB has been administered to the student after age 1 year plus 2 months and 24 days, no further doses are required.
  • When the student is 4 years and 361 days of age or older, this vaccine is not required.

Students are considered compliant when the following conditions have been met:

  • the student receives one dose of the Hib vaccine,
  • the student is at least 38 days old,

Students are considered compliant when the following conditions have been met:

  • the student is at least 66 days old,
  • the student receives the second dose of the Hib vaccine at least 24 days after the first dose.

3 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 94 days old, 
  • the student receives the third dose of the Hib vaccine at least 24 days after the second dose.

This dose is not required if all previous doses were meningococcal protein conjugate.

4 doses
Compliant
Students are considered compliant when the following conditions have been met:
  • the student is at least 361 days old,
  • the student receives the third dose of the Hib vaccine at least 52 days after the previous dose.

Meningococcal

When Varicella and MMR are not administered at the same time, the second live vaccine (whichever is given second) should be repeated after 28 days. The four day grade period does not apply in this instance.

For student entering 6th grade as of July 1st of current year:

For the 2014-2015 calendar year, when the student is in grades 7-9 and has not received a Meningococcal (MCV4 or MenACWY) dose, the student is considered Non-Compliant.
  • When the student is in grades 7-9 and they have had a Meningococcal (MCV4 or MenACWY) dose, they display as No-Requirement.
For the 2015-2016 calendar year, when the student is in grades 7-10 and has not received a Meningococcal (MCV4 or MenACWY) dose, the student is considered Non-Compliant.
  • When the student is in grades 7-10 and they have had a Meningococcal (MCV4 or MenACWY) dose, they display as No-Requirement.
For the 2016-2017 calendar year, when the student is in grades 7-11 and has not received a Meningococcal (MCV4 or MenACWY) dose, the student is considered Non-Compliant.
  • When the student is in grades 7-11 and they have had a Meningococcal (MCV4 or MenACWY) dose, they display as No-Requirement.
For the 2016-2017 calendar year, when the student is in grades 7-12 and has not received a Meningococcal (MCV4 or MenACWY) dose, the student is considered Non-Compliant.
  • When the student is in grades 7-12 and they have had a Meningococcal (MCV4 or MenACWY) dose, they display as No-Requirement.

Booster 16+ 

Compliant

The student is considered compliant if the student student is at least 16 years (192 months) old and has received 2 doses of the Meningococcal (MCV4 or MenACWY) vaccine.

Over 16 shot

Compliant

The student is considered compliant if the following conditions have been met:

  • student is at least 16 years (192 months) old,
  • student has received 1 dose of the Meningococcal (MCV4 or MenACWY) vaccine, and
  • the difference between the last dose and the student's birthdate is greater than or equal to 192 months.

1 dose 16+

Compliant

The student is considered compliant if the following conditions have been met:

  • student is no older than 16 years (192 months) old, 
  • the student is in state grade 6-11, and
  • the student has only received 1 dose of the Meningococcal (MCV4 or MenACWY) vaccine.

Hepatitis A

RuleRequirements
No Requirements under 1Students under the age of 1 year (12 months) old are not required to receive any doses of the Hepatitis A (Hep A) vaccine.

2 doses

Compliant

Students are considered compliant if the following conditions have been met:

  • student is at least 12 months old, but no older than 216 months,
  • the student has received 2 doses of the Hepatitis A vaccine, 
  • the difference between the first dose and the student's birthdate is greater than 12 months, and
  • the difference between the second dose and the previous is at least 6 months with a 4 day tolerance.
