The following information describes vaccine compliance rules defined by the New Jersey State Department of Health.

DTaP

Rule	Requirements
before 3 months No Requirement	Students 3 months old and younger are not required to receive any doses of the DTaP vaccine.
1 dose Compliant	Students are compliant when: they are at least 3 months old but no older than 4 months old, they receive 1 dose of the DT, Tdap, Td, or DTaP vaccine, and the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4-day tolerance.
2 doses Compliant	Students are compliant when: they are at least 5 months old but no older than 6 months old, they receive 2 doses of the DT, Tdap, Td, or DTaP vaccine, the difference between the second and previous dose is greater than or equal to 28 days with a 4-day tolerance, and the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4-day tolerance.
3 doses Compliant	Students are compliant when: they are at least 7 months old but no older than 18 months old, they receive 3 doses of the DT, Tdap, Td, or DTaP vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4-day tolerance, the difference between the second and previous dose is greater than or equal to 28 days with a 4-day tolerance, and the difference between the third and previous dose is greater than or equal to 28 days with a 4-day tolerance.
4 doses under 4 years Compliant	Students are compliant when: they are at least 19 months old but no older than 47 months old, they receive 4 doses of the DT, Tdap, Td, or DTaP vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4-day tolerance, the difference between the second and previous dose is greater than or equal to 28 days with a 4-day tolerance, the difference between the third and previous dose is greater than or equal to 28 days with a 4-day tolerance, and the difference between the fourth and previous dose is greater than 6 months with a 4-day tolerance.
4 doses Compliant	Students are compliant when: they are at least 48 months old but no older than 83 months old, they receive 4 doses of the DT, Tdap, Td, or DTaP vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4-day tolerance, the difference between the second and previous dose is greater than or equal to 28 days with a 4-day tolerance, the difference between the third and previous dose is greater than or equal to 28 days with a 4-day tolerance, the difference between the fourth and previous dose is greater than 6 months with a 4-day tolerance, and the difference between the fourth dose and the student's birthdate is greater than or equal to 48 months with a 4-day tolerance.
5 doses Compliant	Students are compliant when: they are at least 48 months old but no older than 83 months old, they receive 5 doses of the DT, Tdap, Td, or DTaP vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4-day tolerance, the difference between the second and previous dose is greater than or equal to 28 days with a 4-day tolerance, the difference between the third and previous dose is greater than or equal to 28 days with a 4-day tolerance, the difference between the fourth and previous dose is greater than 6 months with a 4-day tolerance, and the difference between the fifth and previous dose is greater than or equal to 6 months with a 4-day tolerance.
3 doses age 7+ Compliant	Students are compliant when: they is at least 84 months old, they receive 3 doses of the DT, Tdap, Td, or DTaP vaccine, the difference between the first dose and the student's birthdate is greater than or equal to 42 days with a 4-day tolerance, the difference between the second and previous dose is greater than or equal to 28 days with a 4-day tolerance, and the difference between the third and previous dose is greater than or equal to 28 days with a 4-day tolerance.
5th doses In Progress	Students are in progress when: they receive 4 doses of the DT, Tdap, Td, or DTaP vaccine, and the difference between the last dose and the effective date is less than or equal to 6 months with a 0-day tolerance. This means that when a student has 4 doses and the last dose is less than or equal to 6 months from the effective date and the student is not compliant, the student is considered in progress.
4th dose In Progress	Students are in progress when: they receive 3 doses of the DT, Tdap, Td, or DTaP vaccine, and the difference between the last dose and the effective date is less than or equal to 6 months with a 0-day tolerance. This means that when a student has 3 doses and the last dose is less than or equal to 6 months from the effective date and the student is not compliant, the student is considered in progress.
3rd dose age 7+ In Progress	Students are in progress when: they are at least 84 months old, they receive 2 doses of the DT, Tdap, Td, or DTaP vaccine, and the difference between the last dose and the effective date is less than or equal to 6 months with a 0-day tolerance. This means that when a student has 2 doses and the last dose is less than or equal to 6 months from the effective date and the student is not compliant, the student is considered in progress.
2nd dose In Progress	Students are in progress when: they receive 1 dose of the DT, Tdap, Td, or DTaP vaccine, and the difference between the last dose and the effective date is less than or equal to 28 days with a 0-day tolerance. This means that when a student has 1 dose, and the dose is less than or equal to 6 months from the effective date and the student is not compliant, the student is considered in progress.

18 years and older Compliant	Students 216 months and older are not required to receive any doses of the IPV, OPV, or IPV/OPV combined Polio vaccine.

Rule	Requirements
1 dose Compliant	Students are compliant when: they are no older than 2 months old, they receive 1 dose of the Hepatitis B vaccine.
2 doses Compliant	Students are compliant when: they are at least 3 months old but no older than 18 months old, they receive 2 doses of the Hepatitis B vaccine, the difference between the second and previous dose is greater than or equal to 28 days with a 4-day tolerance.
2 doses age 11+ Compliant	Students are compliant when: they are at least 132 months old, and they receive 2 doses of the Hepatitis B vaccine.
3 doses Compliant	Students are compliant when: they receive 3 doses of the Hepatitis B vaccine, the difference between the second and previous dose is greater than or equal to 28 days with a 4-day tolerance, the difference between the third and previous dose is greater than or equal to 56 days with a 4-day tolerance, the difference between the third and two doses previous is greater than or equal to 112 with a 4-day tolerance.
3rd dose In Progress	Students are in progress when they have received 2 doses of the Hepatitis B vaccine, and the last dose was administered within the last 56 days with a 0-day tolerance.

Rule	Requirements
0-1 months No Requirement	Students 1 month old and younger are not required to receive any doses of the Hib vaccine.
2-11 months Compliant	Students are compliant when: they are at least 2 months old but no older than 11 months old, and they receive 2 doses of the Hib vaccine.
12-59 months Compliant	Students are compliant when: they are at least 12 months old but no older than 59 months old, they receive 1 dose of the Hib vaccine, and the difference between the first dose and the student's birthdate is greater than or equal to 12 months with a 4-day tolerance.
2-59 months Non-compliant	Students aged 2-59 months who have not received any doses of the Hib vaccine are non-compliant.
60 months No Requirement	Students 60 months old and older are not required to receive any Hib doses.

Pneumococcal

Rule	Requirements
0-1 months No Requirement	Students 1 month old and younger are not required to receive any doses of the Pneumococcal vaccine.
2-11 months Compliant	Students are compliant when: they are at least 2 months old but no older than 11 months old, and they receive 2 doses of the Pneumococcal (PPV or PCV) vaccine.
12-59 months Compliant	Students are compliant when: they are at least 12 months old but no older than 59 months old, they receive 1 dose of the Pneumococcal (PPV or PCV) vaccine, and the difference between the first dose and the student's birthdate is greater than or equal to 12 months with a 4-day tolerance.
2-59 months Non-compliant	Students aged 2-59 months who have not received any doses of the Pneumococcal vaccine are non-compliant.
60 months No Requirement	Students 60 months old and older are not required to receive any Pneumococcal doses.

Varicella

Rule	Requirements

5 Year Booster No Requirement	Students are not required to receive any doses of the Tdap vaccine when: they are at least 132 months old, they are in state grade(s) 06, 07, 08, 09, 10, 11, or 12, they receive 1 dose of the DT, Td, or DTaP/DTP vaccine, and they received the dose in the last 59 months with a 0-day tolerance.
5 Year Booster Compliant	Students are compliant when: they are at least 132 months old, they are in state grade(s) 06, 07, 08, 09, 10, 11, or 12, and they receive 1 dose of the Tdap vaccine.
1 dose Compliant	Students are compliant when: they are at least 132 months old, they are in state grade(s) 06, 07, 08, 09, 10, 11, or 12, they receive 1 dose of the DT, Td, or DTaP/DTP vaccine, and the difference between the last dose and the student's birthdate is greater than or equal to 132 months with a 4-day tolerance.

Rule	Requirements
1 dose Compliant	Students are compliant when: they were born on or after 1/1/1997, they are in state grade 06, and they receive 1 dose of the MenACWY or MCV4 Meningococcal vaccine.
Default 6th grade Non-compliant	Students are non-compliant when: they were born on or after 1/1/1997, they are in state grade 06, and they have not received any doses of the MenACWY or MCV4 Meningococcal vaccine.
