Section 504 provides protections for individuals with disabilities to participate in education, programs, activities, employment, etc. Section 504 provides prohibitions against discrimination and applies to service availability, accessibility, delivery, employment, and the administrative activities and responsibilities of organizations receiving Federal financial assistance. The definition for "individuals with disabilities" under Section 504 is more broad than the disability definition for IEPs and the IEP governing process.

Section 504 Detail Fields
Students may not have overlapping Section 504 records. Users must end date the current record before creating a new record.
Start Date
Indicates the date on which the student began receiving Section 504 accommodations.
End Date
Indicates the date on which the student stopped receiving 504 accommodations.
Program Status
Program Status is used to determine the Student's Section 504 status.
Accommodations
Use the Accommodations field to indicate if the student is receiving accommodations.
Exit Reason
Exit Reason indicates the reason the student exited the program.
State Defined Elements
Disability
Indicates the disability for which the student is receiving accommodations.
Initial Did Not Qualify Date
If the student does not qualify for 504 accommodations, use this field to enter the date on which it was determined they did not qualify.
Last Evaluation Date
Indicates the date on which the student was last evaluated. Students are re-evaluated every three years.
Meeting Date
Use the Meeting Date field to enter the date on which the meeting was held to review a student's 504 plan or to determine if a plan was needed for the student.
