The Campus Default IEP is available for users to document special ed information. Editors and fields are listed below. Included here are instructions on entering data into Campus and references to state-defined guidelines.

Editors

The following table lists the editors available on the student's Individual Education Plan, the section(s) of the print format that include the entered information, a description of what the editor is used for and any special considerations and instructions for using the editor.

Education PlanThe Education Plan editor provides student summary information and lists the dates of the plan meetings.
This editor must be saved with the required fields before continuing to other areas of the plan.
Enrollment Status The Enrollment Editor provides a snapshot of the student's enrollment data when the plan is generated.

Clicking Get Special Ed Status from Enrollment will synchronize the information in the editor with the most recent information from the student's Enrollments tool. 


When establishing the Plan Types, three Enrollment editors are available. Users should choose the editor that best meets their needs. 

Student Demographics
The Student Demographics editor populates basic information about the student such as demographic information and student and school address.
Clicking Refresh Student Information will synchronize the information in the editor with the most recent information about the student, from the Demographics, Households, Enrollments and School tools.
Parent/Guardian Demographics 
The Parent/Guardian Demographics Editor populates based on established student/guardian relationships created on the student's Relationships tool or indicated by the guardian checkbox on the Households tool. The editor includes Demographics information for the student's guardian.
Clicking Refresh Guardian Information will synchronize information in the editor with the most recent information from the student's guardian's Demographics and Households tools.
Team Meeting 
The Team Meeting editor lists the team meetings held regarding the student and the participants of the meeting. Team Meetings display based on the Meeting Date.

Reporting Progress 
The Reporting Progress editor is used to document the method and frequency of reporting the student's progress towards annual goals to the parent/guardian(s) of the student.

PLAAFP 
The Present Level of Academic Achievement and Functional Performance (PLAAFP) editor describes the academic, developmental, and functional needs of the student, including the student's current strengths, parental concerns, the student's needs, the results of assessments, and the impact of the student's exceptionality on their progress.

Special Factors 
The Special Factors editor includes information about the student that may impede the student's learning and progress toward achievement of education goals.

Goals and Objectives 
The Goals and Objectives editor lists goals designed to measure the student's progress in general education curriculum. Each goal includes how the goal is measured and how success is determined.

LRE 
The Least Restrictive Environment editor records information related to the student's school environment and interaction with non-disabled peers

SPED Services 
The Special Ed Services editor is used to document the services provided to the student for their special education needs, including Service Provider, Location, Session Length, and Frequency information.

Related Services 
The Related Services editor identifies developmental, corrective, and other supportive services that are required to assist a child who has a disability to benefit from special education. This includes speech-language pathology and audiology services, in addition to other services that may be available.
Only active Services with a Type of Related can be included in this editor.
Extended School Year 
The Extended School Year editor indicates if the student requires services beyond the standard school time, such as beyond school hours or during breaks.

Transition Plan/Grad Statement 
The Transition Plan/Grad Statement editor is used to document the student's needs, plans, and rights in transitioning from school to post-secondary life. This editor includes current interests and course of study and the student's goals relating to education, employment and living skills.

Transition Services 
The Transition Services editor describes any services the student may need for assisting in transition to post-graduation life.

Transition Activities/Strategies 
The Transition Activities/Strategies editor is used to document any transition services and the agency responsible for providing those services/activities.

Dist-Wide Assessments 
The District Wide Assessments editor is used to document the student's participation and/or assessments required for district wide assessments.

Alt. Dist-Wide Assessments 
The Alternative District Wide Assessments editor is used to document the student's participation in alternative assessments if standardized district with assessments are not an option.

IEP Signature Page 
The IEP Signatures editor records agreements from the parent/guardian, the school personnel, and the student on the information contained in the IEP.

ISP Signature Page 
The ISP Signatures editor records agreements from the parent/guardian, the school personnel, and the student on the information contained in the ISP.
This editor only applies to private school plans.
