Report Logic
Each row counts for students in State Grade Levels KG, 01, 07, and 12, unless otherwise indicated. Counts on this report use the ImmComplianceStatus table to obtain the Compliance Status for each student with an active enrollment on the Effective Date.

| Select Grade(s) | The grade level(s) reported. Multiple grades can be selected. Options are: KG, 01, 07, and/or 12. |
| Generate Report | Generates the report for the calendar(s) selected in the Campus toolbar. |

- Enter an Effective Date. The day defaults to today.
- Select which Grade(s) to include on the report.
- Click Generate Report.

| 2. Total Number of Students with Religious Exemptions | The total number of students with a Religious exemption waiver. |
| 3. Total Number of Students with Personal Exemptions | The total number of students with a Personal exemption waiver. |
| 4. Total Number of Exempted Students | The total number of students with waivers (sums C1 + C2 + C3). |

| Total Students = B1 + C4 + D1 + E1 | The total number of students enrolled compared to the total number of vaccine totals for sections B-E. For example, a school has 100 Kindergarteners Enrolled but the sum of B-E is only 88. This field would report as 100 + 88, indicating there are 12 missing records. |
| G. DIPTHERIA, TETANUS, PERTUSSIS (DTaP, DTP, DT, Tdap, Td) | |
| 1. Total Number Complete | The total number of students who are Compliant or No Requirement for DTaP-DTP. |
