Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliances. The following information describes vaccine compliance rules as defined by the Washington Department of Health.

General Compliance

  • Vaccine doses administered less than or equal to 4 days before the minimum interval or age are counted as valid.
  • Doses administered 5 days earlier than the minimum interval or age shall not be counted as valid and must be repeated as age appropriate.

DTaP

Rule

Requirements

5 doses KG

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade(s): KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives five doses of the DTaP vaccine,
  • the student receives their first dose on or after 6 weeks of age,
  • the student receives the fourth dose on or after 1 year of age,
  • the student receives the fifth dose on or after 4 years of age,
  • the difference between the second and previous dose is greater than or equal to 4 weeks, 
  • the difference between the third and previous dose is greater than or equal to 4 weeks,
  • the difference between the fourth and previous dose is greater than or equal to 4 months, and
  • the difference between the fifth and previous dose is greater than or equal to 6 months.

4 doses KG-12

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade(s): KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives four doses of the DTaP vaccine,
  • the student receives their first dose on or after 6 weeks of age,
  • the student receives the fourth dose on or after 4 years of age,
  • the difference between the second and previous dose is greater than or equal to 4 weeks,
  • the difference between the third and previous dose is greater than or equal to 4 weeks, and
  • the difference between the fourth and previous dose is greater than or equal to 4 months.

3 doses 1-12

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade(s): 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives three doses of the DTaP vaccine,
  • the student receives their first dose on or after 7 years of age,
  • the difference between the second and previous dose is greater than or equal to 4 weeks, and
  • the difference between the third and previous dose is greater than or equal to 4 weeks.

Ages 0-2 months

No Requirement

Students 2 months old and younger are not required to receive any doses of the DTaP vaccine.

1 dose 3 - 4 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade(s): PK,
  • the student is at least 3 months, but no older than 4 months old, 
  • the student receives one dose of the DTaP vaccine, and
  • the student receives their first dose on or after 6 weeks of age.

2 doses 5 - 6 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade(s): PK,
  • the student is at least 5 months, but no older than 6 months old, 
  • the student receives two doses of the DTaP vaccine, 
  • the student receives their first dose on or after 6 weeks of age, and
  • the difference between the second and previous dose is greater than or equal to 4 weeks.

3 doses 7 - 18 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade(s): PK,
  • the student is at least 7 months, but no older than 18 months old, 
  • the student receives three doses of the DTaP vaccine, 
  • the student receives their first dose on or after 6 weeks of age, 
  • the difference between the second and previous dose is greater than or equal to 4 weeks, and
  • the difference between the third and previous dose is greater than or equal to 4 week.

4 doses 19 months - 6 years

Students are considered compliant when the following conditions have been met:

  • the student is in state grade(s): PK,
  • the student is at least 19 months, but no older than 6 years old, 
  • the student receives four doses of the DTaP vaccine, 
  • the student receives their first dose on or after 6 weeks of age, 
  • the difference between the second and previous dose is greater than or equal to 4 weeks, 
  • the difference between the third and previous dose is greater than or equal to 4 week, and
  • the difference between the fourth and previous dose is greater than or equal to 4 months.
5 doses before KG 7yr and older
Compliant
Students are considered compliant when the following conditions have been met:
  • the student is in state grade(s): PK,
  • the student is at least 7 years old, 
  • the student receives five doses of the DTaP vaccine, 
  • the student receives their first dose on or after 6 weeks of age, 
  • the student receives the fourth dose on or after 1 year of age,
  • the student receives the fifth dose on or after 4 years of age, 
  • the difference between the second and previous dose is greater than or equal to 4 weeks, 
  • the difference between the third and previous dose is greater than or equal to 4 week, 
  • the difference between the fourth and previous dose is greater than or equal to 4 months, and
  • the difference between the fifth and previous dose is greater than or equal to 6 months.

A student is considered to be In Process when the student is not compliant and any of the following are true:

  • when either of the first 2 doses were given within the past 4 weeks.
  • when the 3rd dose was given within the past 4 months.
  • when the 4th dose was given within the past 6 months.

Tdap

Rule

Requirements

Grade 7 under 10 yrs
No Requirement
Students in state grade: 07 who are no older than 119 months are not required to receive any doses of the Tdap vaccine.
Grade 8-12 under 7 yrs
No Requirement
Students in state grade: 08, 09, 10, 11, 12 who are no older than 83 months are not required to receive any doses of the Tdap vaccine.
1 dose Grade 7 10 yr+
Compliant
Students are considered compliant when the following conditions have been met:
  • the student is at least 120 months old,
  • the student is in state grade: 07,
  • the student receives one dose of the Tdap vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 120 months with a 4 day tolerance.
1 dose required grades 8-12 7+ yr
Compliant
Students are considered compliant when the following conditions have been met:
  • the student is at least 84 months old,
  • the student is in state grade: 08, 09, 10, 11, 12
  • the student receives one dose of the Tdap vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 84 months with a 4 day tolerance.
1 dose grades 7-12
Non-Compliant
Students are considered non-compliant when the following conditions have been met:
  • the student is in state grade: 07, 08, 09, 10, 11, 12, and
  • the student has not received any doses of the Tdap vaccine.

Hepatitis B

Rule

Requirement

Ages 0-2 months

No Requirement

Students 2 months old and younger are not required to receive any doses of the Hepatitis B vaccine.

1 Dose

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 3 months, but no older than 4 months old, and
  • the student receives one dose of the Hepatitis B vaccine.

2 Doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 5 months, but no older than 18 months old, 
  • the student receives two doses of the Hepatitis B vaccine, and
  • the difference between the second and previous dose is greater than or equal to 4 weeks.

3 Doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student receives three doses of the Hepatitis B vaccine,
  • the student receives the last dose on or after 24 weeks of age,
  • the difference between the second and previous dose is greater than or equal to 4 weeks, 
  • the difference between the third and previous dose is greater than or equal to 8 weeks, and
  • the difference between the third and first dose is greater than or equal to 16 weeks.
3 Doses grades 7-12
Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade(s): 07, 08, 09, 10, 11, 12,
  • the student receives three doses of the Hepatitis B vaccine,
  • the difference between the second and previous dose is greater than or equal to 28 days, 
  • the difference between the third and previous dose is greater than or equal to 56 days, and
  • the student receives the last dose on or after 168 days of age.

2 Adult Doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 11 years old or older, 
  • the student receives two adult doses of the Hepatitis B vaccine, 
  • the difference between the second and previous dose is greater than or equal to 4 months, and
  • the two doses are received by the student between the ages of 11 and 15 years old.

A student is considered to be In Process when the student is not compliant and any of the following are true:

  • when the 1st dose was given within the past 4 weeks.
  • when the 2nd dose was given within the past 8 weeks.
  • when the student has been given 2 doses with the 1st dose within the past 16 weeks.

MMR

Rule

Requirements

2 Doses KG and higher

Compliant

Students are considered compliant when the following conditions have been met:

  • the student receives two doses of the MMR vaccine,
  • the student receives the first dose on or after 12 months of age, and
  • the difference between the second and previous dose is greater than or equal to 4 weeks.

Ages 0-15 months 

No Requirement

Students 15 months old and younger are not required to receive any doses of the MMR vaccine.

1 Dose

Compliant

Students are considered compliant when the following conditions have been met:

  • the student receives one dose of the MMR vaccine,
  • the student receives the dose between 16 months and 6 years of age, and
  • the student receives the first dose on or after 12 months of age.

2 Doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student receives two doses of the MMR vaccine,
  • the student is 7 years old or older and not yet enrolled in KG,
  • the student receives the first dose on or after 12 months of age, and
  • the difference between the second and previous dose is greater than or equal to 4 weeks.

A student is considered to be In Process when the student is not compliant and the 1st dose was given within the past 4 weeks.

Varicella

Rule

Requirements

Grades 7-12

No Requirement

Students in state grade(s): 07, 08, 09, 10, 11, 12 are not required to receive any doses of the Varicella vaccine.

1 Dose Grade 6

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade: 06,
  • the student receives one dose of the Varicella vaccine, and
  • the student receives the first dose on or after 12 months of age.

2 Doses Grades KG-5

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade: KG, 01, 02, 03, 04, 05,
  • the student receives two doses of the Varicella vaccine,
  • the student receives the first dose on or after 12 months of age, and
  • the difference between the second and previous dose is greater than or equal to 4 weeks.

Ages 0-15 months

No Requirement

 Students 15 months old and younger are not required to receive any doses of the Varicella vaccine.

1 Dose

Compliant

Students are considered compliant when the following conditions have been met:

  • the student receives one dose of the Varicella vaccine
  • the student receives the dose between 16 months and 6 years of age, and
  • the student receives the first dose on or after 12 months of age.

2 Doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is 7 years old or older and not yet enrolled in KG,
  • the student receives two doses of the Varicella vaccine,
  • the student receives the first dose on or after 12 months of age, and
  • the difference between the second and previous dose is greater than or equal to 4 weeks.

A student is considered to be In Process when the student is not compliant and the first dose was given within the past 3 months.

Polio

Rule

Requirements

4 Doses KG-2

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade: KG,
  • the student receives four doses of the Polio vaccine,
  • the student receives the first dose on or after 6 weeks of age,
  • the student receives the fourth dose on or after 4 years of age,
  • the difference between the second and previous dose is greater than or equal to 4 weeks, 
  • the difference between the third and previous dose is greater than or equal to 4 week, and
  • the difference between the fourth and previous dose is greater than or equal to 6 months.
3 Doses KG-2
Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade: KG,
  • the student receives three  doses of the IPV, OPV, or IPV,OPV Polio vaccine,
  • the student receives the first dose on or after 42 days of age,
  • the student receives the third dose on or after 4 years of age,
  • the difference between the second and previous dose is greater than or equal to 28 days, and
  • the difference between the third and previous dose is greater than or equal to 6 months.

4 Doses Grades 3-12

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade: 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives four doses of the Polio vaccine,
  • the student receives the first dose on or after 6 weeks of age,
  • the difference between the second and previous dose is greater than or equal to 4 weeks,
  • the difference between the third and previous dose is greater than or equal to 4 weeks, and
  • the difference between the fourth and previous dose is greater than or equal to 6 months.

3 Doses Grades 3-12

Students are considered compliant when the following conditions have been met:

  • the student is in state grade: 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives three doses of the Polio vaccine,
  • the student receives the third dose on or after 4 years of age,
  • the student receives the first dose on or after 6 weeks of age,
  • the difference between the second and previous dose is greater than or equal to 4 weeks, and
  • the difference between the third and previous dose is greater than or equal to 4 weeks.

Over 18

No Requirement

Students 18 years old or older are not required to receive any doses of the Polio vaccine.

Ages 0-2 months

No Requirement

Students 2 months old or younger are not required to receive any doses of the Polio vaccine.

1 Dose Age 3-4 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 3 months, but no older than 4 months old,
  • the student receives one dose of the Polio vaccine, and
  • the student receives the first dose on or after 6 weeks of age.

2 Doses Ages 5-18 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 5 months, but no older than 18 months old,
  • the student receives two doses of the Polio vaccine, 
  • the student receives the first dose on or after 6 weeks of age, and
  • the difference between the second and previous dose is greater than or equal to 4 weeks.

3 Doses 19 months-6 years

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 19 months, but no older than 6 years old,
  • the student receives three doses of the Polio vaccine, 
  • the student receives the first dose on or after 6 weeks of age, 
  • the difference between the second and previous dose is greater than or equal to 4 weeks, and
  • the difference between the third and previous dose is greater than or equal to 4 weeks.

4 Doses Ages 7yr+

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 19 months, but no older than 6 years old,
  • the student receives four doses of the Polio vaccine, 
  • the student receives the first dose on or after 6 weeks of age, 
  • the student receives the fourth dose on or after 4 years of age,
  • the difference between the second and previous dose is greater than or equal to 4 weeks, 
  • the difference between the third and previous dose is greater than or equal to 4 weeks,
  • the difference between the fourth and previous dose is greater than or equal to 6 months.

A student is considered to be In Process when the student is not compliant and any of the following are true:

  • when the 1st or 2nd dose was given within the past 4 weeks.
  • when the 3rd dose was given within the past 8 weeks.
  • when the student has had 3 doses and is under 4 years of age.

Pneumococcal

RuleRequirements

Grades K-12

No Requirement

Students in grade(s): K, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12 are not required to receive any doses of the Pneumococcal PPV or PCV vaccine.

Ages 0-2 months

No Requirement

Students 2 months old and younger are not required to receive any doses of the Pneumococcal PPV or PCV vaccine.

1 Dose Ages 3-4 months

No Requirements

Students are considered compliant when the following conditions have been met:

  • the student is at least 3 months, but no older than 4 months old,
  • the student receives one dose of the Pneumococcal PPV or PCV vaccine, and
  • the student receives the first dose on or after 6 weeks of age.

2 Doses Ages 5-6 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 5 months, but no older than 6 months old,
  • the student receives two doses of the Pneumococcal PPV or PCV vaccine,
  • the student receives the first dose on or after 6 weeks of age, and
  • the difference between the second and previous dose is greater than or equal to 4 weeks.

3 Doses Ages 7-15 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 7 months, but no older than 15 months old,
  • the student receives three doses of the Pneumococcal PPV or PCV vaccine, 
  • the student receives the first dose on or after 6 weeks of age, 
  • the difference between the second and previous dose is greater than or equal to 4 weeks, and
  • the difference between the third and previous dose is greater than or equal to 4 weeks.

4 Doses Ages 16 months - 5 years

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 16 months, but no older than 5 years old,
  • the student receives four doses of the Pneumococcal PPV or PCV vaccine, 
  • the student receives the first dose on or after 6 weeks of age, 
  • the student receives the fourth dose on or after 12 months of age,
  • the difference between the second and previous dose is greater than or equal to 4 weeks, 
  • the difference between the third and previous dose is greater than or equal to 4 weeks, and
  • the difference between the fourth and previous dose is greater than or equal to 8 weeks.

2 Doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student receives two doses of the Pneumococcal PPV or PCV vaccine,
  • the student receives the first dose on or after 12 months of age, and
  • the difference between the second and previous dose is greater than or equal to 8 weeks.

1 Dose

Compliant

Students are considered compliant when the following conditions have been met:

  • the student receives one dose of the Pneumococcal PPV or PCV vaccine, and
  • the student receives the first dose on or after 24 months of age.

Ages 6 years and older

No Requirement

Student 6 years old and older are not required to receive any doses of the Pneumococcal PPV or PCV vaccine.

A student is considered to be In Process when the student is not compliant and any of the following are true:

  • when the 1st or 2nd dose was given within the past 4 weeks.
  • when the 3rd dose was given within the past 8 weeks.
  • when the student has had 3 doses and is under 12 months of age.

Hib

Rule

Requirements

Grades K-12

No Requirement

Students in grade(s): K, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12 are not required to receive any doses of the Hib vaccine.

Ages 0-2 months

No Requirement

Students 2 months old and younger are not required to receive any doses of the Hib vaccine.

1 Dose Ages 3-4 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 3 months, but no older than 4 months old,
  • the student receives one dose of the Hib vaccine, and
  • the student receives the first dose on or after 6 weeks of age.

2 Doses Ages 5-6 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 5 months, but no older than 6 months old,
  • the student receives two doses of the Hib vaccine, 
  • the student receives the first dose on or after 6 weeks of age, and
  • the difference between the second and previous dose is greater than or equal to 4 weeks.

3 Doses Ages 7-15 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 7 months, but no older than 15 months old,
  • the student receives three doses of the Hib vaccine, 
  • the student receives the first dose on or after 6 weeks of age, 
  • the difference between the second and previous dose is greater than or equal to 4 weeks, and
  • the difference between the third and previous dose is greater than or equal to 4 weeks.

4 Doses Ages 16 months - 5 years

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 16 months, but no older than 5 years old,
  • the student receives four doses of the Hib vaccine,

2 Doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student receives two doses of the Hib vaccine,
  • the student receives the first dose on or after 12 months of age, and
  • the difference between the second and previous dose is greater than or equal to 4 weeks.

1 Dose

Compliant

Students are considered compliant when the following conditions have been met:

  • the student receives one dose of the Hib vaccine, and
  • he student receives the first dose on or after 15 months of age.

Ages 6 years and older

No Requirement

.Students 6 years old and older are not required to receive any doses of the Hib vaccine.

A student is considered to be In Process when the student is not compliant and any of the following are true:

  • when the 1st or 2nd dose was given within the past 4 weeks.
  • when the 3rd dose was given within the past 8 weeks.
  • when the student has had 3 doses and is under 12 months of age.
