Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliances. The following information describes vaccine compliance rules as defined by the Washington Department of Health.

General Compliance

Vaccine doses administered less than or equal to 4 days before the minimum interval or age are counted as valid.
Doses administered 5 days earlier than the minimum interval or age shall not be counted as valid and must be repeated as age appropriate.

DTaP, DTP

Rule	Grade/Age	Requirements
5 doses - KG	KG-12	1st dose on or after 6 weeks of age. 4th dose on or after 1 year of age. 5th dose on or after 4 years of age. At least 4 weeks between doses 1 and 2. At least 4 weeks between doses 2 and 3. At least 4 months between doses 3 and 4. At least 6 months between doses 4 and 5.
4 doses - KG-12	KG-12	1st dose on or after 6 weeks of age. 4th dose on or after 4 years of age. At least 4 weeks between doses 1 and 2. At least 4 weeks between doses 2 and 3. At least 4 months between doses 3 and 4.
3 doses - 1-12	1-12	1st dose on or after 7 years of age. At least 4 weeks between doses 1 and 2. At least 4 weeks between doses 2 and 3.
No Requirement	Ages 0-2 months
1 dose 3 - 4 months	PK: Age 0-5 in any Pre-K program	1st dose on or after 6 weeks of age.
2 doses 5 - 6 months	PK: Age 0-5 in any Pre-K program	1st dose on or after 6 weeks of age. At least 4 weeks between doses 1 and 2.
3 doses 7 - 18 months	PK: Age 0-5 in any Pre-K program	1st dose on or after 6 weeks of age. At least 4 weeks between doses 1 and 2. At least 4 weeks between doses 2 and 3.
4 doses 19 months - 6 years	PK: Age 0-5 in any Pre-K program	1st dose on or after 6 weeks of age At least 4 weeks between doses 1 and 2. At least 4 weeks between doses 2 and 3. At least 4 months between doses 3 and 4.
5 doses - before KG 7y and older	PK: Age 0-5 in any Pre-K program	1st dose on or after 6 weeks. 4th dose on or after 1 year of age. 5th dose on or after 4 years of age. At least 4 weeks between doses 1 and 2. At least 4 weeks between doses 2 and 3. At least 4 months between doses 3 and 4. At least 6 months between doses 4 and 5.

A student is considered to be In Process if the student is not compliant and any of the following are true:

If either of the first 2 doses were given within the past 4 weeks.
If the 3rd dose was given within the past 4 months.
If the 4th dose was given within the past 6 months.

Tdap

Rule	Requirements
Grade 7 under 10 yrs No Requirement	Students in state grade: 07 who are no older than 119 months are not required to receive any doses of the Tdap vaccine.
Grade 8-12 under 7 yrs No Requirement	Students in state grade: 08, 09, 10, 11, 12 who are no older than 83 months are not required to receive any doses of the Tdap vaccine.
1 dose Grade 7 10 yr+ Compliant	Students are considered compliant if the following conditions have been met: the student is at least 120 months old, the student is in state grade: 07, the student receives one dose of the Tdap vaccine, and the difference between the last dose and the student's birthdate is greater than 120 months with a 4 day tolerance.
1 dose required grades 8-12 7+ yr Compliant	Students are considered compliant if the following conditions have been met: the student is at least 84 months old, the student is in state grade: 08, 09, 10, 11, 12 the student receives one dose of the Tdap vaccine, and the difference between the last dose and the student's birthdate is greater than 84 months with a 4 day tolerance.
1 dose grades 7-12 Non-Compliant	Students are considered non-compliant if the following conditions have been met: the student is in state grade: 07, 08, 09, 10, 11, 12, and the student has not received any doses of the Tdap vaccine.

Hepatitis B

Rule	Requirement
No Requirement	Ages 0-2 months
1 Dose	Ages 3-4 months
2 Doses	Ages 5-18 months At least 4 weeks between doses 1 and 2.
3 Doses	Last does on or after 24 weeks of age. At least 4 weeks between doses 1 and 2. At least 8 weeks between doses 2 and 3. At least 16 weeks between doses 1 and 3.
3 Doses	State grades 7-12 At lease 28 days between doses 1 and 2. At least 56 days between doses 2 and 3. Last dose on or after 168 days of age.
2 Adult Doses	Ages 11 years and older Doses given at least 4 months apart. Doses given between the ages of 11 and 15 years.

A student is considered to be In Process if the student is not compliant and any of the following are true:

If the 1st dose was given within the past 4 weeks.
If the 2nd dose was given within the past 8 weeks.
If the student has been given 2 doses with the 1st dose within the past 16 weeks.

MMR

Rule	Requirements
2 Doses - KG and higher	1st dose on or after 12 months of age. At least 4 weeks between doses.
No Requirement	Ages 0-15 months of age.
1 Dose	Ages 16 months - 6 years of age. 1st dose on or after 12 months of age.
2 Doses	Ages 7 years and older but not yet enrolled in KG. 1st dose on or after 12 months of age. At least 4 weeks between doses.

A student is considered to be In Process if the student is not compliant and the 1st dose was given within the past 4 weeks.

Varicella

Rule	Requirements
No Requirement	Grades 7-12
1 Dose - Grade 6	Grade 6 1st dose on or after 12 months of age.
2 Doses - Grades KG-5	1st dose on or after 12 months of age. At least 4 weeks between doses.
No Requirement	Ages 0-15 months of age.
1 Dose	Ages 16 months to 6 years of age. 1st dose on or after 12 months of age.
2 Doses	Ages 7 years and older but not yet enrolled in KG. 1st dose on or after 12 months of age. At least 4 weeks between doses.

A student is considered to be In Process if the student is not compliant and the first dose was given within the past 3 months.

Polio

Rule	Grade/Age	Requirements
4 Doses - KG-2	Grades KG-2	1st dose on or after 6 weeks of age. At least 4 weeks between doses 1 and 2. At least 4 weeks between doses 2 and 3. At least 6 months between doses 4 and 4. 4th dose on or after 4 years of age.
3 Doses - KG-2	Grades KG-2	3 doses of IPV, OPV, or IPV,OPV. 1st does on or after 42 days of age. At least 28 days between doses 1 and 2. At least 6 months between doses 2 and 3. Last dose on or after 4 years of age.
4 Doses - Grades 3-12	Grades 3-12	1st dose on or after 6 weeks of age. At least 4 weeks between doses 1 and 2. At least 4 weeks between doses 2 and 3. At least 6 months between doses 3 and 4.
3 Doses - Grades 3-12	Grades 3-12	1st dose on or after 6 weeks of age. At least 4 weeks between doses 1 and 2. At least 4 weeks between doses 2 and 3. 3rd dose on or after 4 years of age.
No Requirement	Over 18 years of age
No Requirement	Ages 0-2 months
1 Dose	Ages 3-4 months	1st dose on or after 6 weeks of age.
