Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliances. The following information describes vaccine compliance rules as defined by the Indiana State Department of Health.

As of the 1214 Release Packs (April 2012), vaccine compliancy has been updated to have a Live Vaccine option. The following vaccines have been updated to include the Live Vaccine:

  • MMR
  • MMRV
  • Measles
  • Rubella
  • Mumbs
  • Flu
  • Varicella

General Compliance

  • Vaccine doses administered less than or equal to four days before the minimum interval or age are counted as valid.
  • Doses administered five days earlier than the minimum interval or age are not counted as valid doses and should be repeated as age-appropriate.
  • No requirements exist for state grade PW (preschool 0-2 years).

The following tables describe all Indiana immunization requirements per vaccine.

Hepatitis B

Students are considered compliant if the following conditions have been met:

  • the student is at least 132 months (11 years) old, and
  • the student receives two doses of the Hepatitis B - 2 Dose (HepB-2) vaccine.

3 doses 

Compliant

Students are considered compliant if the following conditions have been met:

  • the student receives three doses of the Hepatitis B vaccine,
  • the difference between the second dose and the first valid dose is greater than 28 days with a 4 day tolerance,
  • the difference between the last dose and the second valid dose is greater than 56 days with a 4 day tolerance,
  • the difference between the last dose and the first valid dose is greater than 112 days with a 4 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 168 days with a 4 day tolerance.

3rd dose

Students are considered in progress if the following conditions have been met:

  • the student receives two doses of the Hepatitis B vaccine, and
  • the difference between the last dose and the effective date is less than 56 days with a 0 (zero) day tolerance.

3rd dose

Students are considered in progress if the following conditions have been met:

  • the student receives two doses of the Hepatitis B vaccine, and
  • the difference between the second to last and the effective date is less than 112 days with a 0 (zero) day tolerance.

2nd dose

Students are considered in progress if the following conditions have been met:

  • the student receives one dose of the Hepatitis B vaccine, and
  • the difference between the last dose and the effective date is less than 28 days with a 0 (zero) day tolerance.

2 dose HepB-2

Students are considered in progress if the following conditions have been met:

  • the student is at least 132 months (11 years) old, 
  • the student receives one dose of the Hepatitis B - 2 Dose (HepB-2) vaccine, and

Hepatitis A

There are no immunization requirements for PK and 3-12 Grade levels.

  • the student receives two doses of the Hepatitis A vaccine,
  • the difference between the second dose and the previous is greater than 6 months with a 4 day tolerance, and
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

Students are considered in progress if the following conditions have been met:

  • the student is in state grade: KG: Kindergarten, 01, 02, 03, 04, 05, 06, 12,
  • the student receives one dose of the Hepatitis A vaccine, and
  • the difference between the last dose and the effective date is less than 5 months with a 0 (zero) day tolerance.

1 dose

Active Start Date: 07/01/2020

Students are considered in progress if the following conditions have been met:

  • the student receives one dose of the Hepatitis A vaccine, and
  • the difference between the last dose and the effective date is less than 5 months with a 1 day tolerance.

K-7; 12

Non-compliant

Active End Date: 06/30/2020

Students are considered non-compliant if the following conditions have been met:

  • the student is in state grade: KG: Kindergarten, 01, 02, 03, 04, 05, 06, 12, and
  • the student has not received any doses of the Hepatitis A vaccine.

PreK-12 2020-2021

Non-compliant

Active Start Date: 07/01/2020

Students are considered non-compliant if the student has not received any doses of the Hepatitis A vaccine.

DTaP

Doses of Tdap are also acceptable to fulfill these requirements, except for the 4 dose rule for PK students. There are no requirements for students age 0-1 month.

Students age 0-1 month old are not required to receive any doses of the DTaP vaccine.

1 dose Age 2-3 months 

Compliant 

Students are compliant if the following conditions have been met:

  • the student is at least 2 months, but no older than 3 months old,
  • the student receives one dose of the DT, DTaP, Td, or Tdap vaccine, and
  • the difference between the first dose and student's birthdate greater than 42 days with a 4 day tolerance.

2 doses Age 4-5 months 

Compliant 

Students are compliant if the following conditions have been met:

  • the student is at least 4 months, but no older than 5 months old,
  • the student receives two dose of the DT, DTaP, Td, or Tdap vaccine, 
  • the difference between the first dose and student's birthdate greater than 42 days with a 4 day tolerance, and
  • the difference between the second and the previous dose is greater than 28 days with a 4 day tolerance.

3 doses Age 6-14 months 

Compliant 

Students are compliant if the following conditions have been met:

  • the student is at least 6 months, but no older than 14 months old,
  • the student receives three dose of the DT, DTaP, Td, or Tdap vaccine, 
  • the difference between the first dose and student's birthdate greater than 42 days with a 4 day tolerance, 
  • the difference between the second and the previous dose is greater than 28 days with a 4 day tolerance, and
  • the difference between the third and the previous dose is greater than 28 days with a 4 day tolerance.

4 doses under 4 

Compliant 

Students are compliant if the following conditions have been met:

  • the student is at no older than 47 months old,
  • the student receives four dose of the DT, DTaP, Td, or Tdap vaccine, 
  • the difference between the first dose and student's birthdate greater than 42 days with a 4 day tolerance, 
  • the difference between the second and the previous dose is greater than 28 days with a 4 day tolerance, 
  • the difference between the third and the previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the fourth dose and the previous is greater than 6 months with a 4 day tolerance, and
  • the difference between the fourth dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

4 doses 

Compliant 

Students are compliant if the following conditions have been met:

  • the student is at least 48 months (4 years) old,
  • the student receives four dose of the DT, DTaP, Td, or Tdap vaccine, 
  • the difference between the first dose and student's birthdate greater than 42 days with a 4 day tolerance, 
  • the difference between the second and the previous dose is greater than 28 days with a 4 day tolerance, 
  • the difference between the third and the previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the fourth dose and the previous is greater than 6 months with a 4 day tolerance, and
  • the difference between the fourth dose and the student's birthdate is greater than 48 months with a 4 day tolerance.

5 doses 

Compliant 

Students are compliant if the following conditions have been met:

  • the student is at least 48 months (4 years) old,
  • the student receives four dose of the DT, DTaP, Td, or Tdap vaccine, 
  • the difference between the first dose and student's birthdate greater than 42 days with a 4 day tolerance, 
  • the difference between the second and the previous dose is greater than 28 days with a 4 day tolerance, 
  • the difference between the third and the previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the fourth dose and the previous is greater than 6 months with a 4 day tolerance, and
  • the difference between the fifth dose and the previous is greater than 6 months with a 4 day tolerance.

3 doses 7+ 

Compliant 

Students are compliant if the following conditions have been met:

  • the student is at least 84 months (7 years) old,
  • the student receives three dose of the DT, DTaP, Td, or Tdap vaccine, 
  • the difference between the first dose and student's birthdate greater than 12 months with a 4 day tolerance, 
  • the difference between the second and the previous dose is greater than 28 days with a 4 day tolerance, 
  • the difference between the third and the previous dose is greater than 6 months with a 4 day tolerance, and
  • the difference between the third dose and the student's birthdate is greater than 84 months with a 4 day tolerance.

4 doses 7+ 

Compliant 

Students are compliant if the following conditions have been met:

  • the student is at least 84 months (7 years) old,
  • the student receives four dose of the DT, DTaP, Td, or Tdap vaccine, 
  • the difference between the first dose and student's birthdate less than 11 months with a 4 day tolerance, 
  • the difference between the second and the previous dose is greater than 28 days with a 4 day tolerance, 
  • the difference between the third and the previous dose is greater than 28 days with a 4 day tolerance,
  • he difference between the fourth and the previous dose is greater than 6 months with a 4 day tolerance,, and
  • the difference between the fourth dose and the student's birthdate is greater than 84 months with a 4 day tolerance.

3 doses

Students are considered in progress if the following conditions have been met:

  • the student receives three dose of the DT, DTaP, Td, or Tdap vaccine, and
  • the difference between the last dose and the effective date is less than 5 months with a 1 day tolerance.

2 doses 7+

Students are considered in progress if the following conditions have been met:

  • the student is at least 84 months (7 years) old,
  • the student receives two dose of the DT, DTaP, Td, or Tdap vaccine, and
  • the difference between the last dose and the effective date is less than 5 months with a 1 day tolerance.

  • the student receives one dose of the DT, DTaP, Td, or Tdap vaccine, and

Tdap

One dose is required for state grades 06, 07, 08, 09, 10, 11, 12, given on or after 10 years of age, with a 4 day tolerance.

Meningococcal

One dose is required for grades 6 and above.

  • the student is in state grade: 06, 07, 08, 09, 10, 11, and
  • the student receives one dose of the Meningococcal (MCV4 or MenACWY) vaccine.

2 doses

Compliant

Students are considered  compliant if the following conditions have been met:

  • the student is in state grade: 12, and
  • the student receives two doses of the Meningococcal (MCV4 or MenACWY) vaccine.

1 dose 16 yrs

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is in state grade: 12,
  • the student receives one dose of the Meningococcal (MCV4 or MenACWY) vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 192 months with a 4 day tolerance.

Grades 6-12 

Non-compliant

Students are considered non-compliant if the following conditions have been met:

  • the student is in state grade: 06, 07, 08, 09, 10, 11, 12, and
  • the student hasn't received any doses of the Meningococcal (MCV4 or MenACWY) vaccine.

  • the student is in state grade: PK: Pre-kindergarten,
  • the student receives one dose of the Measles vaccine, and
    • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance OR
  • the student receives one dose of the Mumps vaccine, and
    • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance, OR
  • the student receives one dose of the Rubella vaccine, and
    • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

2MMR

Students are considered compliant if the following conditions have been met:

  • the student is in state grade: KG: Kindergarten, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,

  • the student receives two doses of the Measles vaccine, and
    • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
    • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, OR

  • the student is in state grade: KG: Kindergarten, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives four doses of the IPV, OPV, or IPV/OPV vaccine,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, 
  • the difference between the third dose and the previous is greater than 28 days with a 4 day tolerance, 
  • the difference between the fourth dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the last dose was received before 08/09/2009.

  • the student receives four doses of the IPV, OPV, or IPV/OPV vaccine,
  • the difference between the fourth dose and the student's birthdate is greater than 48 months with a 4 day tolerance, and
  • the difference between the fourth dose and the previous is greater than 6 months with a 4 day tolerance.

3 doses IPV

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is in state grade: KG: Kindergarten, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives four doses of the IPV, or IPV/OPV vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, 
  • the difference between the third dose and the previous is greater than 6 months with a 4 day tolerance, and
  • the difference between the third dose and the student's birthdate is greater than 48 months with a 4 day tolerance.

3 doses OPV

Students are considered compliant if the following conditions have been met:

  • the student is in state grade: KG: Kindergarten, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives four doses of the OPV, or IPV/OPV vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, 
  • the difference between the third dose and the previous is greater than 6 months with a 4 day tolerance, and
  • the difference between the third dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
