Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliances. The following information describes vaccine compliance rules as defined by the Nebraska Department of Health.

General Compliance

  • Vaccine doses administered less than or equal to 4 days before the minimum interval or age are considered valid.
  • Doses administered 5 days before the minimum interval or age are not considered valid and must be repeated as age-appropriate.

Hepatitis B

  • Students between the ages of 2 and 5 years who are enrolled in a school based program are required to have 3 doses of the pediatric Hepatitis B vaccine. 
  • Students entering school for the first time (K or 1st), students entering 7th grade and transfer students from outside Nebraska (regardless of grade) are required to have 3 doses of the pediatric Hepatitis B vaccine OR 2 doses of the adolescent vaccine if between the ages of 11 and 15 years.
  • Students not described above (grades 1-6 and 8-12) are required to have 3 doses of the pediatric Hepatitis B vaccine OR 2 doses of the adolescent vaccine if between the ages of 11 and 15 years.

DTaP

Td, Tdap, DT and DTAP vaccines are also acceptable for these requirements.

Students 23 months old and younger are not required to receive any doses of the DTaP vaccine.
3 doses
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: HK: Kindergarten prog of <1032 instructional prog hrs, KG: Kindergarten 1032 or more instructional prog hours, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives three doses of the DTaP vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
K-12
Non-Compliant
Students are considered non-compliant if the following conditions have been met:
  • the student is in state grade: HK: Kindergarten prog of <1032 instructional prog hrs, KG: Kindergarten 1032 or more instructional prog hours, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, and
  • the student has received no doses of the DTaP vaccine.
2-5 years
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 24 months, but no older than 71 months old, and
  • the student receives four doses of the DTaP vaccine.

Tdap

RuleRequirements
1 dose 8-12
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: 08, 09, 10, 11, 12,
  • the student receives one dose of the Tdap vaccine, and
  • the difference between the dose and the student's birthdate is greater than 84 months with a 4 day tolerance.
1 dose 7th
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: 07,
  • the student receives one dose of the Tdap vaccine, and
  • the difference between the dose and the student's birthdate is greater than 84 months with a 4 day tolerance.
No dose 7-12
Non-compliant
Students are considered non-compliant if the following conditions have been met:
  • the student is in state grade: 07, 08, 09, 10, 11, 12, and
  • the student has not received any doses of the Tdap vaccine.

Hib

RuleRequirements
K(after age 5) - 12
No Requirement
Students who meet the following conditions are not required to receive any doses of the Hib vaccine: 
  • the student is at least 61 months old, and
  • the student is in state grade: HK: Kindergarten prog of <1032 instructional prog hrs, KG: Kindergarten 1032 or more instructional prog hours, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12.
2-4 years - 3 doses
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is 24 months, but no older than 59 months old, and
  • the student receives three doses of the Hib vaccine.
2-4 years - 1 dose after 15 mos
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is 24 months, but no older than 59 months old, 
  • the student receives one dose of the Hib vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 15 months with a 4 day tolerance.
2-4 years
Non-Compliant
Students are considered non-compliant if the following conditions have been met:
  • the student is 24 months, but no older than 59 months old, and
  • the student receives no doses of the Hib vaccine.

MMR

No Requirement
Students 23 months old and younger are not required to receive any doses of the MMR vaccine.
2 doses MMR
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: HK: Kindergarten prog of <1032 instructional prog hrs, KG: Kindergarten 1032 or more instructional prog hours, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives two doses of the MMR or MMRV vaccine,
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
2 doses M,M,R
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: HK: Kindergarten prog of <1032 instructional prog hrs, KG: Kindergarten 1032 or more instructional prog hours, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives two doses of the Measles vaccine, and
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, 
    • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, 
  • the student receives two doses of the Mumps vaccine, and
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance,
    • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the student receives two doses of the Rubella vaccine, and
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, 
    • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
2 doses - Mix
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: HK: Kindergarten prog of <1032 instructional prog hrs, KG: Kindergarten 1032 or more instructional prog hours, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives two doses of the Measles vaccine and MMR or MMRV vaccine, and
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, 
    • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, 
  • the student receives two doses of the Mumps vaccine and MMR or MMRV vaccine, and
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance,
    • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the student receives two doses of the Rubella vaccine and MMR or MMRV vaccine, and
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, 
    • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
K-12
Non-Compliant
Students are considered non-compliant if the following conditions have been met:
  • the student is in state grade: HK: Kindergarten prog of <1032 instructional prog hrs, KG: Kindergarten 1032 or more instructional prog hours, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives no doses of the MMR or MMRV vaccine.
2-5 years - MMR
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 24 months, but no older than 71 months old,
  • the student receives one dose of the MMR or MMRV vaccine, and
  • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
2-5 years - M,M,R
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 24 months, but no older than 71 months old,
  • the student receives one dose of the Measles vaccine, and
    • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance,
  • the student receives one dose of the Mumps vaccine, and
    • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance,
  • the student receives one dose of the Rubella vaccine, and
    • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

Varicella

  • Students between the ages of 2 and 5 who are enrolled in a school based program are required to have 1 dose of varicella or MMRV given on or after 12 months of age. Written documentation (including year) of varicella disease from parent, guardian or health care provider will be accepted.
  • Students entering school for the first time (K or 1st), students entering 7th grade and transfer students from outside Nebraska (regardless of grade) are required to have 1 dose of varicella or MMRV if given on or after 12 months of age and prior to 13 years of age (end 6/30/2011). If given over 13 years of age, 2 doses of varicella (not MMRV) are required, separated by at least one month.  Written documentation (including year) of varicella disease from parent, guardian or health care provider will be accepted.
  • Students not described above (grades 1-6 and 8-12) are required to have 1 dose of varicella. Written documentation (including year) of varicella disease from parent, guardian or health care provider will be accepted.

Polio

All students are required to have 3 doses of the polio vaccine.

Pneumococcal

RuleRequirements
Default K (after age 5) - 12
No Requirement
Students are not required to receive any doses of the Pneumococcal PCV or PPV vaccine if the following conditions have been met:
  • the student is at least 61 months old, and
  • the student is in state grade: HK: Kindergarten prog of <1032 instructional prog hrs, KG: Kindergarten 1032 or more instructional prog hours, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12.
Default over 59 months
No Requirement
Students 60 months old and older are not required to receive any doses of the Pneumococcal PCV or PPV vaccine.
Default under 24 months
No Requirement
Students 23 months old and younger are not required to receive any doses of the Pneumococcal PCV or PPV vaccine.
1 dose
Compliant
Students are considered compliant if the following conditions have been met:
  • the student receives one dose of the Pneumococcal PCV or PPV vaccine, and
  • the difference between the dose and the student's birthdate is greater than 15 months with a 4 day tolerance.
4 doses
Compliant
Students are considered compliant if the following conditions have been met:
  • the student receives four doses of the Pneumococcal PCV or PPV vaccine.
