Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliance rules. The following information describes vaccine compliance rules as defined by the Illinois State Department of Health.

General Compliance

  • Compliance for Measles, Mumps and Rubella is indicated in the status for MMR. Compliance or exemption in each of these counts towards compliance in MMR.
  • If dose spacing is 3 months or less, a month is 28 days.
  • If dose spacing is 4 months or more, a month is a full calendar month. 
  • The day tolerance for all rules is 4 days.

Immunization Rules

The following tables describe Illinois immunization requirements per vaccine. 

Illinois immunization rules are hard-coded and cannot be modified.

Hepatitis B

Students are considered compliant if the following conditions have been met:
  • the student is at least 2 months, but no older than 3 months old,
  • the student receives two doses of the Hepatitis B vaccine, and
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
3 doses PK
Compliant
Students are considered compliant if the following conditions have been met:
  • the student receives three doses of the Hepatitis B vaccine, 
  • the student is in state grade 00: Birth to 3, 14: Pre-K, 
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 168 days with a 4 day tolerance.
3 doses 6-12
Compliant
Students are considered compliant if the following conditions have been met:
  • the student receives three doses of the Hepatitis B vaccine,
  • the student is in state grade(s): 06, 07, 08, 09, 10, 11, 12,
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the last dose and the second valid dose is greater than 56 days with a 4 day tolerance,
  • the difference between the last dose and the first valid dose is greater than 112 days with a 4 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 168 days with a 4 day tolerance.
2 doses

Students are considered in progress if the following conditions have been met:
  • the student receives two doses of the Hepatitis B vaccine, and
  • the difference between the last dose and the effective date is less than 56 days with a 0 (zero) day tolerance.

  • the student receives one dose of the Hepatitis B vaccine, and
  • the difference between the last dose and the effective date is less than 28 days with a 0 (zero) day tolerance.

DTap

4 doses required 

New entries 

The student is considered compliant if the following conditions are met:

  • the student is enrolled in a grade lower than grade 2
  • the difference between the first and second dose is at least 4 weeks,
  • the difference between the second and third dose is at least 6 months, and
  • the third dose is on or after the student's 4th birthday.

3 doses required 

Existing entries 

The student is considered compliant if the following conditions are met:

  • The student is enrolled in a grade higher than grade K,
  • the difference between the first and second dose is at least 4 weeks,
  • the difference between the second and third dose is at least 6 months, and
  • the third dose is on or after the student's 4th birthday.

Hib

There are no requirements for students under 56 days of age or over 5 years of age.

Students age 0-1 month old are not required to receive any doses of the Haemophilus influenza, type B (Hib) vaccine.

Age 5 years and older

No Requirement

Student 60 months (5 years) old and older are not required to receive any doses of the Haemophilus influenza, type B (Hib) vaccine.

1 dose Age 2-3 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 2 months, but no older than 3 months old,
  • the student receives one dose of the Haemophilus influenza, type B (Hib) vaccine, and
  • the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance.

2 doses Age 4-5 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 4 months, but no older than 5 months old,
  • the student receives two doses of the Haemophilus influenza, type B (Hib) vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, and
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance.

3 doses <7 months 1st shot

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is no older than 11 months old,
  • the student receives three doses of the Haemophilus influenza, type B (Hib) vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the first dose and the student's birthhdate is less than 6 months with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the difference between the third dose and the previous is greater than 28 days with a 4 day tolerance.

3 doses 7-11 months 1st shot

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is no older than 11 months old,
  • the student receives three doses of the Haemophilus influenza, type B (Hib) vaccine,
  • the difference between the first dose and the student's birthdate is greater than 7 months with a 4 day tolerance,
  • the difference between the first dose and the student's birthhdate is less than 11 months with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the difference between the third dose and the previous is greater than 56 days with a 4 day tolerance.

3 doses Hib 3 dose 7-11 months 1st shot

Compliant

Students are considered compliant if the following conditions have been met:

  • the student receives 3 doses of the Hib 3 dose series (Hib-3),
  • the difference between the first dose and the student's birthdate is greater than 7 months with a 4 day tolerance,
  • the difference between the first dose and the student's birthhdate is less than 11 months with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the third dose and the previous is greater than 56 days with a 4 day tolerance, and
  • the difference between the third dose and the student's birthdate is reater than 12 months with a 4 day tolerance.

3 doses after 12-59 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 12 months, but no older than 59 months old,
  • the student receives three doses of the Haemophilus influenza, type B (Hib) vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the first dose and the student's birthhdate is less than 11 months with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the third dose and the previous is greater than 56 days with a 4 day tolerance, and
  • the difference between the third dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

2 doses Hib 3 after 1 year

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 12 months, but no older than 59 months old,
  • the student receives 2 doses of the Hib 3 dose series (Hib-3),
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the second dose and the previous is greater than 56 days with a 4 day tolerance.

4 doses

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 12 months old,
  • the student receives four doses of the Haemophilus influenza, type B (Hib) vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the first dose and the student's birthhdate is less than 11 months with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the second dose and the student's birthdate is less than 11 months with a 4 day tolerance,
  • the difference between the third dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the third dose and the student's birthdate is less than 11 months with a 4 day tolerance, and
  • the difference between the fourth dose and the previous is greater than 56 days with a 4 day tolerance.

2 doses 12-14 months 1st shot

Compliant

Students are considered compliant if the following conditions have been met:

  • the student receives two doses of the Haemophilus influenza, type B (Hib) vaccine,
  • the difference between the second dose and the previous is greater than 56 days with a 4 day tolerance.
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the first dose and the student's birthhdate is less than 14 months with a 4 day tolerance.

1 dose 15 months 1st shot

Compliant

Students are considered compliant if the following conditions have been met:

  • the student receives one dose of the Haemophilus influenza, type B (Hib) vaccine, and
  • the difference between the dose and the student's birthdate is greater than 15 months with a 4 day tolerance.

1 dose In Progress before 1 yr

In Progress

Students are considered in progress if the following conditions have been met:

  • the student receives one dose of the Haemophilus influenza, type B (Hib) vaccine,
  • the difference between the dose and the effective date is less than 28 days with a 0 (zero) day tolerance, and
  • the difference between the dose and the student's birthdate is less than 11 months with a 0 (zero) day tolerance.

1 dose In Progress after 1 yr

In Progress

Students are considered in progress if the following conditions have been met:

  • the student receives one dose of the Haemophilus influenza, type B (Hib) vaccine,
  • the difference between the dose and the effective date is less than 56 days with a 0 (zero) day tolerance, and
  • the difference between the dose and the student's birthdate is greater than 12 months with a 0 (zero) day tolerance.

2 doses in Progress

In Progress

Students are considered in progress if the following conditions have been met:

  • the student is at least 7 months old,
  • the student receives two doses of the Haemophilus influenza, type B (Hib) vaccine, and
  • the difference between the last dose and the effective date is less than 56 days with a 0 (zero) day tolerance.

2 doses Hib 3 dose in Progress before 1 year

Students are considered in progress if the following conditions have been met:

  • the student is at least 7 months, but no older than 12 months old,
  • the student receives 2 doses of the Hib 3 dose series (Hib-3),
  • the difference between the last dose and the effective date is less than 56 days with a 0 (zero) day tolerance.

Measles

Rule

Condition

1 dose required

The student is considered compliant if the following condition is met:

  • The dose is received on or after 1 year of age for any student enrolled in a state grade of 14 and equal to or older than 2 years of age.

2 doses required

The student is considered compliant if the following conditions are met:

  • The doses are received on or after the first birthday and at least 4 weeks apart for any student enrolled in a grade of KG or higher.

Mumps

Rule

Condition

1 dose required

The student is considered compliant if the following condition is met:

  • The dose is received on or after the 1st birthday for any student 2 years of age and older.
2 doses required

As of the 2014-15 school year, 2 doses are required for all grade levels.

  • the first dose must be administered on or after the student's first birthday, and
  • the second dose must be administered no less than four weeks after the first dose.

Rubella

Rule

Condition

1 dose required

The student is considered compliant if the following condition is met:

  • The dose is received on or after the 1st birthday for any student 2 years of age and older.
2 doses required

As of the 2014-15 school year, 2 doses are required for students in grades KG-12.

  • the first dose must be administered on or after the student's first birthday, and
  • the second dose must be administered no less than four weeks after the first dose.

Varicella

Rule

Condition

1 dose required

The student is considered compliant if the following condition is met:

  • The dose is received on or after the first birthday for students 2 years of age or older.
2 doses required

The student is considered compliant if the following condition is met:

  • student is in any grade,
  • the first dose must be administered on or after the student's first birthday, and
  • the difference between the second and first dose is 4 weeks.

Polio

No requirement for students 23 months or younger. 

Rule

Condition

3 doses required

The student is considered compliant if the following conditions are met:

  • the doses are received between ages 24 months and 47 months,
  • the student receives three doses of the Polio OPV, Polio IPV/OPV, or Polio IPV, Polio IPV/OPV vaccine, and
  • the difference between doses is 28 days.

3 doses required

The student is considered compliant for Polio OPV or Polio IPV if the following conditions are met:

  • the student is enrolled in a Pre-K grade level,
  • the student receives three doses of the Polio OPV, Polio IPV/OPV, or Polio IPV, Polio IPV/OPV vaccine, and
  • the difference between doses is 28 days.
3 doses required

The student is considered compliant for Polio OPV or Polio IPV if the following conditions are met:

  • the student is enrolled in grade K, 
  • the student is at least 24 months (2 years) old,
  • the student receives three doses of the Polio OPV, Polio IPV/OPV, or Polio IPV, Polio IPV/OPV vaccine,
  • the difference between the first and second doses is 28 day,
  • the difference between the second and third doses is 28 days, and
  • the last dose is administered on or after the student's 4th birthday.

4 doses required

The student is considered compliant for Polio OPV or IPV if the following conditions are met:

  • the student is enrolled in grades K-12,
  • the student is at least 24 months (2 years) old,
  • the student receives four doses of the Polio OPV, Polio IPV/OPV, or Polio IPV, Polio IPV/OPV vaccine,
  • the difference between the first and second doses is 28 day,
  • the difference between the second and third doses is 28 days, 
  • the difference between the third and fourth doses is 28 days, and
  • the last dose is administered on or after the student's 4th birthday.
4 doses required

The student is considered compliant for Polio OPV if the following conditions are met:

  • the student receives four doses of the Polio OPV, Polio IPV/OPV vaccine,
  • the difference between the first and second doses is 28 day,
  • the difference between the second and third doses is 28 days, 
  • the last dose is administered on or after the student's 4th birthday, and
  • the fourth dose is at least 6 months after third dose.
4 doses required

The student is considered compliant for Polio IPV when the following conditions are met:

  • the student receives four doses of the Polio IPV, Polio IPV/OPV vaccine,
  • the difference between the first and second doses is 28 day,
  • the difference between the second and third doses is 28 days, 
  • the last dose is administered on or after the student's 4th birthday, and
  • the fourth dose is at least 6 months after third dose.

Tdap

RuleRequirements

1 Dose Required

The student is considered compliant if the following conditions are met:

  • the dose is received before the student is in grade 6 or higher, 
  • the receives one dose of the Tdap, DTaP-DTP, DT, or Td vaccine,
  • the student is in grade 6 or high, and
  • the difference between the last dose and the student's birthdate is greater than 120 months with a 4 day tolerance.

Pneumococcal

All doses have a 4 day tolerance unless otherwise specified.

Rule

Age 0-1 month

No Requirement

Student 1 month old or less are not required to receive any doses of the Pneumococcal vaccine.

Age 5 and older

No Requirement 

Student 60 months (5 years) old and older are not required to receive any doses of the Pneumococcal vaccine.

1 dose required age 2-3 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 2 months old and no older than 3 months old,
  • the student receives 1 dose of the Pneumococcal vaccine, and
  • the difference between the dose and the student's birthdate is greater than or equal to 42 days.

2 doses required age 4-5 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 4 months old and no older than 5 months old,
  • the student receives 2 doses of the Pneumococcal vaccine,
  • the difference between the dose and the student's birthdate is greater than or equal to 42 days, and
  • the difference between the second dose and the previous is greater than or equal to 28 days.

3 doses 6-11 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 6 months old and no older than 11 months old,
  • the student receives 3 doses of the Pneumococcal vaccine,
  • the difference between the dose and the student's birthdate is greater than or equal to 42 days,
  • the difference between the second dose and the previous is greater than or equal to 28 days, and
  • the difference between the third dose and the previous is greater than or equal to 28 days.
2 doses Age 6-11 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 6 months old and no older than 11 months old,
  • the student receives 2 doses of the Pneumococcal vaccine,
  • the difference between the first dose and the student's birthdate is greater than or equal to 42 days,
  • the difference between the second and previous dose is greater than or equal to 28 days, and
  • the difference between the second dose and the student's birthdate is greater than or equal to 7 months.

4 doses 1 year

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 12 months old,
  • the student receives 4 doses of the Pneumococcal vaccine,
  • the difference between the dose and the student's birthdate is greater than 42 days,
  • the difference between the second dose and the previous is greater than or equal to 28 days,
  • the difference between the third dose and the previous is greater than or equal to 28 days, 
  • the difference between the third dose and the student's birthdate is less than 12 months, and
  • the difference between the fourth dose and the previous is greater than or equal to 56 days.

3 doses 1 year and older

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 12 months old,
  • the student receives 3 doses of the Pneumococcal vaccine,
  • the difference between the first dose and the student's birthdate is less than 12 months,
  • the difference between the first dose and the student's birthdate is greater than or equal to 42 days,
  • the difference between the second dose and the previous is greater than or equal to 28 days,
  • the difference between the third dose and the previous is greater than or equal to 56 days, and
  • the difference between the third dose and the student's birthdate is greater than or equal to 12 months.

2 doses 1 year and older

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 12 months old,
  • the student receives 2 doses of the Pneumococcal vaccine,
  • the difference between the first dose and the student's birthdate is greater than or equal to 12 months, and
  • the difference between the second dose and the previous is greater than or equal to 56 days.

1 dose Age over 24 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 24 months old,
  • the student receives 1 dose of the Pneumococcal vaccine, and
  • the difference between the dose and the student's birthdate is greater than or equal to 24 months.
In ProgressStudents are considered in progress if they have received one dose of the Pneumococcal vaccine and the difference between the dose and the effective date is less than or equal to 28 days with a 0 (zero) day tolerance.

Meningococcal MCV4

Students in grades PK-5 and grades 11 do not need this vaccine.

Students are considered compliant if the following conditions have been met:
  • the student is at least 132 months old,

  • the student receives one dose of the Meningococcal vaccine, and
  • the difference between the first dose and the student's birthdate is greater than 120 months with a 0 (zero) day tolerance.
2 doses Grade 12
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: 12,
  • the student receives two doses of the Meningococcal vaccine, 
  • the difference between the second and previous dose is greater than 56 days with a 4 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 192 months with a 4 day tolerance.
1 dose after age 16
Compliant
Students are considered compliant if the following conditions have been met:
  • the student receives one dose of the Meningococcal vaccine, and
  • the difference between the first dose and the student's birthdate is greater than 192 months with a 4 day tolerance.
1 dose
In Progress
Students are considered in progress if the following conditions have been met:
  • the student receives one dose of the Meningococcal vaccine, and
  • the difference between the first dose and the effective date is less than 56 days with a 4 day tolerance.
No dose
Non-compliant
Students in state grade: 06, 07, 08, 09, 10, 11, or 12, and are 132 months old who have not received any doses of the Meningococcal vaccine are considered non-compliant.
