Health vaccines and their associated compliance listings are based on state guidelines. Vaccine requirements for Connecticut are hard-coded. The following information describes vaccine compliance rules as defined by the Connecticut Department of Public Health.

General Compliance

  • DTaP vaccine is not given on or after the 7th birthday and may be given for all doses in the primary series.
  • Tdap can be given in lieu of Td vaccine for children 7 years and older unless contraindicated. Tdap is only licensed for one dose.
  • Hib is not required for children 5 years of age or older.
  • Pneumococcal is required for all Pre-K and K students born on or after 1/1/2007 and less than 5 years of age.
  • Hepatitis A is required for all Pre-K and K students born on or after 1/1/2007.
  • Second MMR for school year 2012-2013 applies to all students in Grades K-12.

DTaP

Rule

Requirements

3 doses K-12

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade: KH: Kindergarten, Half Day, KE: Kindergarten, Extended Day, KF: Kindergarten, Full Day, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives three doses of the DT, Td, Tdap, or DTaP-DTP vaccine,
  • the difference between the first dose and the student's birthdate is greater than 84 months with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the difference between the third dose and the previous is greater than 6 months with a 4 day tolerance.

4 doses K-12

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade: KH: Kindergarten, Half Day, KE: Kindergarten, Extended Day, KF: Kindergarten, Full Day, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives four doses of the DT, Td, Tdap, or DTaP-DTP vaccine,
  • the difference between the last dose and the student's birthdate is greater than 48 months with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the third dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the difference between the fourth dose and the previous is greater than 6 months with a 4 day tolerance.

4 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 18 months old,
  • the student receives four doses of the DT, Td, Tdap, or DTaP-DTP vaccine,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the third dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the difference between the fourth dose and the previous is greater than 6 months with a 4 day tolerance.

3 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 7 months, but no older than 17 months old, and
  • the student receives three doses of the DT, Td, Tdap, or DTaP-DTP vaccine.

2 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 5 months, but no older than 6 months old, and
  • the student receives two doses of the DT, Td, Tdap, or DTaP-DTP vaccine.

1 dose

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 3 months, but no older than 4 months old, and
  • the student receives one dose of the DT, Td, Tdap, or DTaP-DTP vaccine.

No Requirement

Students age 0-2 months old are not required to receive any doses of the DT, Td, Tdap, or DTaP-DTP vaccine.

Tdap

Rule

Requirements

1 dose Tdap

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade: 07, 08, 09, 10, 11, 12,
  • the student receives one dose of the Tdap or DTaP-DTP vaccine, and
  • the difference between the dose and the student's birthdate is greater than 84 months with a 4 day tolerance.

Preschool - Grade 6

No Requirement

Students in state grades: P3: 3 year old Pre-K, KH: Kindergarten, Half Day, KE: Kindergarten, Extended DayFull Day, 01, 02, 03, 04, 05, 06, are not required to receive any doses of the Tdap or DTaP-DTP vaccine.

Hepatitis B

Rule

Requirements

3 doses

Compliant

Students are considered compliant when  the following conditions have been met:

  • the student receives three doses of the Hepatitis B (Hep B) vaccine,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the third dose and the second valid dose is greater than 56 days with a 4 day tolerance,
  • the difference between the third dose and the two doses previous is greater than 112 days with a 4 day tolerance, and
  • the difference between the third dose and the student's birthdate is greater than 168 days with a 4 day tolerance.

2 doses 

In Progress

The student is considered in progress when the following conditions have been met:

  • the student receives two doses of the Hepatitis B vaccine, and
  • the difference between the second dose and the effective date is less than or equal to 56 days with a 0 (zero) day tolerance.

1 dose

In Progress

The student is considered in progress when the following conditions have been met:

  • the student receives 1 dose of the Hepatitis B vaccine, and
  • the difference between the dose and the effective date is less than or equal to 28 days with a 0 day tolerance.

Meningococcal

Rule

Requirements

1 dose grades 7-12 

Compliant

The student is considered compliant when the following conditions have been met:

  • the student receives 1 dose of the Meningococcal vaccine, and
  • the student is in state grade 07, 08, 09, 10, 11, 12.

Default 7-12 Non-Compliant

The student is considered non-compliant if they are in State Grades 07, 08, 09, 10, 11, 12 and has not received any doses of the Meningococcal vaccine.

MMR

Rule

Requirements

2 MMR K-12

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is in state grade: KH: Kindergarten, Half Day, KE: Kindergarten, Extended Day, KF: Kindergarten, Full Day, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives two doses of the Measles-Mumps Rubella (MMR) or Measles-Mumps-Rubella-Varicella (MMRV) vaccine,
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance.

2M2M2R K-12

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is in state grade: KH: Kindergarten, Half Day, KE: Kindergarten, Extended Day, KF: Kindergarten, Full Day, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives two doses of the Measles vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
    • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, 
  • the student receives two doses of the Mumps vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
    • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, 
  • the student receives two doses of the Rubella vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
    • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance.

1MMR1M1M1R K-12

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is in state grade: KH: Kindergarten, Half Day, KE: Kindergarten, Extended Day, KF: Kindergarten, Full Day, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives one dose of the Measles vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, OR
  • the student receives one dose of the Mumps vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, OR
  • the student receives one dose of the Rubella vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, OR
  • the student receives two doses of the Measles, Measles-Mumps Rubella (MMR) or Measles-Mumps-Rubella-Varicella (MMRV) vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and

  • the student receives two doses of the Mumps, Measles-Mumps Rubella (MMR) or Measles-Mumps-Rubella-Varicella (MMRV) vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
    • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, OR
  • the student receives two doses of the Rubella, Measles-Mumps Rubella (MMR) or Measles-Mumps-Rubella-Varicella (MMRV) vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a4 day tolerance, and
    • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance.

1 dose 16 months

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is at least 16 months old,
  • the student receives one dose of the Measles-Mumps Rubella (MMR) or Measles-Mumps-Rubella-Varicella (MMRV) vaccine, and
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

1M1M1R

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is at least 16 months old,
  • the student receives one dose of the Measles vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, OR
  • the student receives one dose of the Mumps vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, OR
  • the student receives one dose of the Rubella vaccine,
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

Age 0-15 months

No requirement

Students age 0-15 months old are not required to receive any doses of the Measles-Mumps Rubella (MMR) or Measles-Mumps-Rubella-Varicella (MMRV) vaccine.

Varicella

Rule

Requirements

2 doses 7-12

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is in state grade: 07, 08, 09, 10, 11, 12,
  • the student receives two doses of the Varicella or Measles-Mumps-Rubella-Varicella (MMRV) vaccine, and
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance 
  • the difference between the first dose and the student's birthdate is greater than 156 months with a 4 day tolerance,

2 doses

Compliant

The student is considered compliant when the following conditions have been met:

  • the student receives two doses of the Varicella or Measles-Mumps-Rubella-Varicella (MMRV) vaccine, and
  • the difference between the second dose and the previous is greater than 3 months with a 4 day tolerance, and
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

Ages 0-18 months

No Requirement

Students age 0-18 months old are not required to receive any doses of the Varicella or Measles-Mumps-Rubella-Varicella (MMRV) vaccine.

1 dose ages 19 months and older

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is at least 19 months old,
  • the student receives one dose of the Varicella or Measles-Mumps-Rubella-Varicella (MMRV) vaccine, and
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

Polio

Rule

Requirements

3 doses K-12

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is in state grade: KH: Kindergarten, Half Day, KE: Kindergarten, Extended Day, KF: Kindergarten, Full Day, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives three doses of the IPV, OPV, or IPV/OPV Polio vaccine,
  • the difference between the last dose and the student's birthdate is greater than 48 months with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the difference between the third dose and the previous is greater than 2 months with a 4 day tolerance.

3 doses Ages 18 months and older

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is at least 18 months old,
  • the student receives three doses of the IPV, OPV, or IPV/OPV Polio vaccine,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the difference between the third dose and the previous is greater than 2 months with a 4 day tolerance.

2 doses Ages 5 months - 17 months

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is at least 5 months, but no older than 17 months old,
  • the student receives two doses of the IPV, OPV, or IPV/OPV Polio vaccine, and
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance.

1 dose Ages 3 months - 4 months

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is at least 3 months, but no older than 4 months old, and
  • the student receives one dose of the IPV, OPV, or IPV/OPV Polio vaccine.

0-2 months

No Requirements

Students age 0-2 months old are not required to receive any doses of the Polio vaccine.

Pneumococcal

Ages 0-2 months

No Requirement

Students age 0-2 months old are not required to receive any doses of the Pneumococcal PPV or PCV vaccine.

Grades K-12

No Requirement

Students age 60 months (5 years) old or older in state grade: KH: Kindergarten, Half Day, KE: Kindergarten, Extended Day, KF: Kindergarten, Full Day, 01, 02, 03, 04, 05, 06, 08, 09, 10, 11, 12, are not required to receive any doses of the Pneumococcal PPV or PCV vaccine.

1 dose Ages 3-4 months

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is at least 3 months, but no older than 4 months old, and
  • the student receives one dose of the Pneumococcal PPV or PCVvaccine.

2 doses Ages 5 months - 6 months

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is at least 5 months, but no older than 6 months old, and
  • the student receives two doses of the Pneumococcal PPV or PCV vaccine.

2 doses Ages 7 months - 15 months

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is at least 7 months, but no older than 15 months old, and
  • the student receives three doses of the Pneumococcal PPV or PCV vaccine.

1 dose after 1st birthday for children less than 5

Compliant

The student is considered compliant when the following conditions have been met:

  • the student receives one dose of the Pneumococcal PPV or PCV vaccine, and
  • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

HIB

Age 5 yrs and older

No requirement

Students 60 months (5 years) old and older are not require to receive any doses of the Haemophilus influenza, type B (Hib) vaccine.

1 dose after 1st birthday for children less than 5

Compliant

The student is considered compliant when the following conditions have been met:

  • the student receives one dose of the Haemophilus influenza, type B (Hib) vaccine, and
  • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

  • the student is in state grade: P3: 3 year old Pre-K, PK: Prekindergarten, 
  • the student is at least 24 months, but no older than 59 months old,
  • the student receives one dose of the Influenza vaccine, and
  • the difference between the last dose and effective date greater than or equal to 11 months with a 0 (zero) day tolerance.

2 doses 

Compliant

The student is considered compliant when the following conditions have been met:

  • the student is in state grade: P3: 3 year old Pre-K, PK: Prekindergarten, 
  • the student is at least 24 months, but no older than 59 months old,
  • the student receives two doses of the Influenza vaccine, 
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the difference between the last dose and the effective date is less than 6 months with a 1 day tolerance.
Default ages 24-59 months
Non-compliant
The student is considered non-compliant when the following conditions have been met:
  • the student is at least 24 months, but no older than 59 months old,
  • the student is in state grade(s): P3: 3 year old Pre-K, PK: Prekindergarten, and
  • the student has not received any doses of the Influenza vaccine.
