Health vaccines and their associated compliance listings are based on state guidelines. Vaccine requirements for Colorado are hard-coded. The following information describes vaccine compliance rules as defined by the Colorado Department of Health.

Colorado follows the CDC ACIP recommendations for vaccinations.

A State Exemption code allows mapping of vaccines to an appropriate exemption code, as follows. These mappings are entered on the Vaccine Exemptions tool.

  • M: Medical
  • N: Non-Medical
  • L: Lab Verified

All immunizations follow the spacing rules as follows:

  • A student who did not have the fourth dose on or after four years should be able to get a fifth dose on or after four years and be compliant if the spacing is correct, but they do not become compliant with a fifth dose on or after four years of age.

General Compliance

  • Vaccine doses administered less than or equal to four days before the minimum interval of age are valid and are considered compliant.
  • Vaccine doses administered less than or equal to five days before the minimum interval of age are NOT valid and are considered NOT compliant.
  • Doses of Varicella and MMR must be given on the same day, or at least 28 days apart.
  • For calculation purposes, 4 weeks = 28 days. Intervals of four months or greater are determined by calendar months.

Immunizations

Hepatitis B

Students enrolled in Grades K-12

Rule

1 dose 0-1 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 1 month old, and
  • the student receives one dose of the Hepatitis B (Hep B) vaccine.

2 doses 2-3 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 2 months, but no older than 3 months old,
  • the student receives two doses of the Hepatitis B (Hep B) vaccine, and
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance.

3 doses

Compliant

Students are considered compliant if the following conditions have been met:

  • the student receives three doses of the Hepatitis B (Hep B) vaccine,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the last dose and the second valid dose is greater than 56 days with a 4 day tolerance,
  • the difference between the last dose and the first valid dose is greater than 112 days with a 4 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 168 days with a 4 day tolerance.

2 doses Hep B-2 Over 11 yrs

Students are considered in progress if the following conditions have been met:

  • the student is at least 132 months old,
  • the student receives two doses of the Hepatitis B - 2 dose (Hep B - 2 Dose) vaccine, and
  • the difference between the second dose and the previous is greater than 4 months with a 4 day tolerance.

1 dose Hep B-2 Over 11 yrs

In Progress

Students are considered in progress if the following conditions have been met:

  • the student is at least 132 months old,
  • the student receives one dose of the Hepatitis B - 2 dose (Hep B - 2 Dose) vaccine, and
  • the difference between the last dose and the effective date is less than 3 months with a 1 day tolerance.

2 doses

In Progress

Students are considered in progress if the following conditions have been met:

  • the student receives two doses of the Hepatitis B (Hep B) vaccine, and
  • the difference between the last dose and the effective date is less than 56 days with a 0 (zero) day tolerance.

1 dose

Students are considered in progress if the following conditions have been met:

  • the student receives one dose of the Hepatitis B (Hep B) vaccine, and
  • the difference between the last dose and the effective date is less than 28 days with a 0 (zero) day tolerance.

Students age 0-1 months old are not required to receive any doses of the DT, Td, Tdap, or DTaP-DTP vaccine.

1 dose Age 2-3 months

Students are considered compliant if the following conditions have been met:

  • the student is at least 2 months, but no older than 3 months old,
  • the student receives one dose of the DT, Td, Tdap, or DTaP-DTP vaccine, and
  • the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance.

2 doses Age 4-5 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 4 months, but no older than 5 months old,
  • the student receives two doses of the DT, Td, Tdap, or DTaP-DTP vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, and
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance.

3 doses Age 6-14 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 6 month, but no older than 14 months old,
  • the student receives three doses of the DT, Td, Tdap, or DTaP-DTP vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the difference between the third dose and the previous is greater than 28 days with a 4 day tolerance.

4 doses under 4

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is no older than 47 months old,
  • the student receives four doses of the DT, Td, Tdap, or DTaP-DTP vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the third dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the fourth dose and the previous is greater than 4 months with a 4 day tolerance, and
  • the difference between the fourth dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

  • the student is at least 84 months old,
  • the student receives three doses of the DT, Td, Tdap, or DTaP-DTP vaccine,
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the third dose and the previous is greater than 6 months with a 4 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 84 months with a 4 day tolerance.

  • the student is at least 84 months old,

  • the student is at least 84 months old,
  • the student receives two doses of the DT, Td, Tdap, or DTaP-DTP vaccine, and
  • the difference between the last dose and the effective date is less than 5 months with a 1 day tolerance.

Students are considered compliant if the following conditions have been met:

  • the student is in state grade: 060, 070, 080, 090, 100, 110, 120, 
  • the student receives one dose of the Tdap or DTaP-DTP vaccine, and
  • the difference between the dose and the student's birthdate is greater than 120 months with a 4 day tolerance.

No doses

Non-compliant

Students in state grade: 060, 070, 080, 090, 100, 110, 120 who have not received any doses of the Tdap or DTaP-DTP vaccine are considered non-compliant.

Hib

Rule

Requirements

Age 0-1 months

No requirement

Students 1 month old or younger are not required to receive any doses of the Haemophilus influenza, type B (Hib) vaccine. 

Age 5 yrs and older

No requirement

Students 60 months (5 years) old and older are not require to receive any doses of the Haemophilus influenza, type B (Hib) vaccine.

1 dose Age 2-3 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 2 months, but no older than 3 months old,
  • the student receives one dose of the Haemophilus influenza, type B (Hib) vaccine, and
  • the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance.

2 doses Age 4-5 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 2 months, but no older than 3 months old,
  • the student receives two doses of the Haemophilus influenza, type B (Hib) vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, and
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance.

3 doses <7 months 1st shot

  • the student is no older than 11 months old,
  • the student receives three doses of the Haemophilus influenza, type B (Hib) vaccine,

  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, and

3 doses 7-11 months 1st shot

Students are considered compliant if the following conditions have been met:

  • the student is no older than 11 months old,
  • the student receives three doses of the Haemophilus influenza, type B (Hib) vaccine,
  • the difference between the first dose and the student's birthdate is greater than 7 months with a 4 day tolerance,
  • the difference between the first dose and the student's birthdate is less than 11 months with a 4 day tolerance,

Students are considered compliant if the following conditions have been met:

  • the student is at least 12 months, but no older than 59 months old,
  • the student receives three doses of the Haemophilus influenza, type B (Hib) vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the first dose and the student's birthdate is less than 11 months with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the third dose and the previous is greater than 56 days with a 4 day tolerance, and
  • the difference between the third dose and the student's birthdate is greater than 12 months.

  • the student is at least 2 months, but no older than 3 months old,
  • the student receives one dose of the Pneumococcal (PPV) vaccine, and

2 doses 12 months

Compliant

Students are considered compliant if the following conditions have been met:

  • the student is at least 12 months old,
  • the student receives two doses of the Pneumococcal (PPV) vaccine,
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the second dose and previous is greater than 56 days with a 4 day tolerance.

  • the student receives one doses of the Pneumococcal (PPV) vaccine, and
  • the difference between the first dose and the student's birthdate is greater than 24 months with a 4 day tolerance
