Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliances. The following information describes vaccine compliance rules as defined by the Utah Department of Health.

DTaP, DTP

Diphtheria-tetanus-pertussis, combined [DTaP, DTP] (code: DTaP-DTP)

RuleRequirements
Age 0-1 months
No Requirement
Students 1 month old and younger are not required to receive any doses of the DTaP, DTP vaccine.
1 dose Age 2-5 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 2 months old, but no older than 5 months old,
  • the student receives one dose of the DTaP/DTP, DT, TD, or Tdap vaccine, and
  • the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance.
2 doses Age 4-8 months 
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 4 months old, but no older than 8 months old,
  • the student receives two doses of the DTaP/DTP, DT, TD, or Tdap vaccine, 
  • the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance, and
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
3 doses Age 6-18 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 6 months old, but no older than 18 months old,
  • the student receives three doses of the DTaP/DTP, DT, TD, or Tdap vaccine, 
  • the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance, 
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, and
  • the difference between the third and previous dose is greater than 28 days with a 4 day tolerance.
4 doses under 7 
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is no older than 83 months,
  • the student is in state grade: PK,
  • the student receives four doses of the DTaP/DTP, DT, TD, or Tdap vaccine, 
  • the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance, 
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, 
  • the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, 
  • the difference between the fourth and previous dose is greater than 4 months with a 4 day tolerance, and
  • the difference between the fourth dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
4 doses 
Compliant
Students are considered compliant if the following conditions have been met:
  • the student receives four doses of the DTaP/DTP, DT, TD, or Tdap vaccine, 
  • the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance, 
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, 
  • the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, 
  • the difference between the fourth and previous dose is greater than 4 months with a 4 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 48 months with a 4 day tolerance.

Compliant
Students are considered compliant if the following conditions have been met:
  • the student receives five doses of the DTaP/DTP, DT, TD, or Tdap vaccine, 
  • the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance, 
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance, 
  • the difference between the third and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the fourth and previous dose is greater than 4 months with a 4 day tolerance, and
  • the difference between the fifth and previous dose is greater than 6 months with a 4 day tolerance.
3 doses 7+
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 84 months (7 years) old,
  • the student receives three doses of the DTaP/DTP, DT, TD, or Tdap vaccine,
  • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance, 
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 84 months with a 4 day tolerance.
4 doses 7+
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 84 months (7 years) old,
  • the student receives four doses of the DTaP/DTP, DT, TD, or Tdap vaccine, 
  • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance, 
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, 
  • the difference between the fourth and previous dose is greater than 6 months with a 4 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 84 months with a 4 day tolerance.

Students are considered in progress if the following conditions have been met:
  • the student receives three doses of the DTaP/DTP, DT, TD, or Tdap vaccine, and
  • the difference between the last dose and the effective date is less than 6 months with a 1 day tolerance.
2 doses 7+

Students are considered in progress if the following conditions have been met:
  • the student is at least 84 months (7 years) old,
  • the student receives two doses of the DTaP/DTP, DT, TD, or Tdap vaccine, and
  • the difference between the last dose and the effective date is less than 6 months with a 1 day tolerance.

Students are considered in progress if the following conditions have been met:

Tdap

Tetanus, Diphtheria and Acellular Pertussis [Tdap] (code: Tdap)

Students are considered compliant if the following conditions have been met:
  • the student is in state grade: 07, 08, 09, 10, 11, 12,
  • the student receives one dose of the Tdap vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 120 months with a 4 day tolerance.
No dose grades 7-12 
Non-compliant
Students are considered non-compliant if the following conditions have been met:
  • the student is in state grade: 07, 08, 09, 10, 11, 12, and
  • the student receives no doses of the Tdap vaccine.

Polio

Students 1 month old and younger are not required to receive any doses of the Polio vaccine.
Age 18+
No Requirement
Students 18 years old and older are not required to receive any doses of the Polio vaccine.

1 dose Age 2-3 months 

Compliant

Students are considered compliant if the following conditions have been met:
  • the student is at least 2 months old, but no older than 3 months old,
  • the student receives one dose of the IPV or combined IPV/OPV Polio vaccine, and
  • the difference between the dose and the student's birthdate is greater than 42 days with a 4 day tolerance.

2 doses Age 4-5 months

Students are considered compliant if the following conditions have been met:
  • the student is at least 4 months old, but no older than 5 months old,
  • the student receives two doses of the IPV or combined IPV/OPV Polio vaccine, 
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, and
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.

Students are considered compliant if the following conditions have been met:
  • the student receives four doses of the IPV or combined IPV/OPV Polio vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, 
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the third and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the fourth and previous dose is greater than 28 days with a 4 day tolerance, and
  • the student receives the last dose before 08/07/2010.

Compliant
Students are considered compliant if the following conditions have been met:
  • the student receives four doses of the IPV or combined IPV/OPV Polio vaccine,
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, 
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the third and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the fourth and previous dose is greater than 6 months with a 4 day tolerance, and
  • the difference between the fourth dose and the student's birthdate is greater than 48 months with a 4 day tolerance.

Students are considered compliant if the following conditions have been met:
  • the student is at least 6 months, but no older than 47 months old,
  • the student receives three doses of the IPV or combined IPV/OPV Polio vaccine,

  • the student is at least 48 months (4 years) old,
  • the student receives three doses of the IPV or combined IPV/OPV Polio vaccine,

  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the third and previous dose is greater than 6 months with a 4 day tolerance, and

Measles Mumps Rubella MMR

Students are considered compliant if the following conditions have been met:
  • the student is in state grade: PK: Pre-kindergarten,
  • the student receives one dose of the Measles, MMR, or MMRV vaccine, and
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, OR
  • the student receives one dose of the Mumps, MMR, or MMRV vaccine, and
    • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, OR
  • the student receives one dose of the Rubella, MMR, or MMRV vaccine, and

Students are considered compliant if the following conditions have been met:
  • the student is in state grade: 00: Kindergarten, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives two doses of the MMR or MMRV vaccine,
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance.

Students are considered compliant if the following conditions have been met:

Students are considered compliant if the following conditions have been met:
  • the student receives three doses of the Hepatitis B vaccine,
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the last and the second valid dose is greater than 56 days with a 4 day tolerance,
  • the difference between the last and the first valid dose is greater than 112 days with a 4 day tolerance, and

Students are considered non-compliant if the following conditions have been met:
  • the student is in state grade: 07, 08, 09, 10, 11, 12, and
  • the student does not receive any doses of the Meningococcal vaccine.

Hib

Haemophilus influenza, type B [Hib] (code: Hib)

Students 1 month old and younger are not required to receive any doses of the Hib vaccine.
Age 5 years and older

Students 60 months (5 years) old and older are not required to receive any doses of the Hib vaccine.

Students are considered compliant if the following conditions have been met:
  • the student is as least 2 months, but no older than 3 months old,
  • the student receives one dose of the Hib or the Hib 3 dose vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 42 days with a 4 day tolerance.
2 doses Age 4-5 months

Students are considered compliant if the following conditions have been met:

Students are considered in progress if the following conditions have been met:
  • the student is as least 7 months, but no older than 12 months olf,

Hepatitis A

  • the student receives two doses of the Hepatitis A vaccine,

  • the difference between the second and previous dose is greater than 6 months with a 4 day tolerance.

Students are considered in progress if the following conditions have been met:
  • the student receives one dose of the Hepatitis A vaccine, and
  • the difference between the first dose and the effective date is less than 6 months with a 1 day tolerance.

Pneumococcal PPV

Students 1 month old and younger are not required to receive any doses of the PPV vaccine.
Ages 5 and older

Students 60 months (5 years) old and older are not required to receive any doses of the PPV vaccine.

  • the student is as least 2 months, but no older than 3 months old,
  • the student receives one dose of the PPV vaccine, and

  • the student is as least 6 months, but no older than 11 months old,
  • the student receives three doses of the PPV vaccine, 
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, and
  • the difference between the second dose and the student's birthdate is less than 6 months with a 4 day tolerance.
4 doses 12 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is as least 12 months old,
  • the student receives four doses of the PPV vaccine, 
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the third and previous dose is greater than 28 days with a 4 day tolerance, and
  • the difference between the fourth dose and the student's birthdate is greater than 56 days with a 4 day tolerance.
3 doses 12 months; 7-11 shot
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is as least 12 months old,
  • the student receives three doses of the PPV vaccine, 
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the third and previous dose is greater than 56 days with a 4 day tolerance,
  • the difference between the second dose and the student's birthdate is greater than 7 months with a 4 day tolerance, 
  • the difference between the second dose and the student's birthdate is less than 11 months with a 4 day tolerance, and
  • the difference between the third dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

Students are considered compliant if the following conditions have been met:
  • the student is as least 12 months old,

  • the student is as least 12 months old,
  • the student receives two doses of the PPV vaccine,
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the second and previous dose is greater than 56 days with a 4 day tolerance.
1 dose over 24 months
Compliant

Students are considered compliant if the following conditions have been met:

  • the student receives one dose of the PPV vaccine, and
  • the difference between the first dose and the student's birthdate is greater than 24 months with a 4 day tolerance.
