Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliances. The following information describes vaccine compliance rules as defined by the Iowa State Department of Health.

General Compliance

  • Vaccine doses administered less than or equal to 4 days before the minimum interval or age are counted as valid.
  • Doses administered 5 days earlier than the minimum interval or age are not counted as valid doses and must be repeated as age-appropriate.

Hepatitis B

For students in grades K-12, 3 doses are required for students 4 years and older. This rule applies to students born on or after July 1, 1994. No requirement for students not yet enrolled in a grade of K or higher.

Students are considered compliant if the following conditions have been met:

  • the student is born on or before 07/01/1994,
  • the student is in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives three doses of the Hepatitis B vaccine,
  • the difference between the second and first valid dose is greater than 28 days with a 4 day tolerance,
  • the difference between the last dose and the second valid dose is greater than 56 days with a 4 day tolerance,
  • the difference between the last dose and the student's birthdate is greater than 168 days with a 4 day tolerance, and
  • the difference between the last dose and the first valid dose is greater than 112 days with a 4 day tolerance. 

DTaP

RuleRequirements

3 doses 7+

Compliant

Students are considered compliant if the following conditions have been met:
  • the student is at least 84 months old,
  • the student receives three doses of the DT, DTaP, Td, or Tdap vaccine,
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
3 Doses
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is born on or after 09/15/2000,
  • the student is in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives three doses of the DT, DTaP, Td, or Tdap vaccine,
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
4 Doses
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives four doses of the DT, DTaP, Td, or Tdap vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
5 Doses
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives five doses of the DT, DTaP, Td, or Tdap vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
K-12 Default
Non-compliant
Students in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, who have not received any doses of the DTaP vaccine are considered non-compliant.
0-3 months
No Requirement
Students 3 months old and younger are not required to receive any doses of the DTaP vaccine.
4-5 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 4 months, but no older than 5 months old, and
  • the student receives one dose of the DT, DTaP, Td, or Tdap vaccine.
6-11 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 6 months, but no older than 11 months old, and
  • the student receives two doses of the DT, DTaP, Td, or Tdap vaccine.
12-18 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 12 months, but no older than 18 months old, and
  • the student receives three doses of the DT, DTaP, Td, or Tdap vaccine.
19 months and older
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 19 months old, and
  • the student receives four doses of the DT, DTaP, Td, or Tdap vaccine.

Hib

60 months and older
No Requirement
Students 60 months old and older are not required to receive any doses of the Haemophilus influenza, type B [Hib] vaccine.
0-3 months
No Requirements
Students 3 months old and younger are not required to receive any doses of the Haemophilus influenza, type B [Hib] vaccine.
4-5 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 4 months, but no older than 5 months old, and
  • the student receives one dose of the Hib vaccine.
6-18 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 6 months, but no older than 18 months old, and
  • the student receives two doses of the Hib vaccine.
19-59 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 19 months, but no older than 59 months old, 
  • the student receives three doses of the Hib vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
12-59 months 1 dose
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 12 months, but no older than 59 months old, 
  • the student receives one dose of the Hib vaccine, and
  • the difference between the dose and the student's birthdate is greater than 15 months with a 4 day tolerance.

MMR

  • the student receives two doses of the MMR or MMRV vaccine,
  • the difference between the second to last dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the last and previous dose is greater than 28 days with a 4 day tolerance.
2 M 2 R
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives two doses of the Measles vaccine, and
    • the difference between the second to last dose and the student's birthdate is greater than 12 months with a 4 day tolerance,
    • the difference between the last and previous dose is greater than 28 days with a 4 day tolerance,
  • the student receives two doses of the Rubella vaccine, and
    • the difference between the second to last dose and the student's birthdate is greater than 12 months with a 4 day tolerance,
    • the difference between the last and previous dose is greater than 28 days with a 4 day tolerance.
1 MMR 1 M 1 RStudents are considered compliant if the following conditions have been met:
  • the student is in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives two doses of the Measles, MMR, or MMRV vaccine, and
    • the difference between the second to last dose and the student's birthdate is greater than 12 months with a 4 day tolerance,
    • the difference between the last and previous dose is greater than 28 days with a 4 day tolerance,
  • the student receives two doses of the Rubella, MMR, or MMRV vaccine, and
    • the difference between the second to last dose and the student's birthdate is greater than 12 months with a 4 day tolerance,
    • the difference between the last and previous dose is greater than 28 days with a 4 day tolerance.
K-12 Default
Non-compliant
Students in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, who have not received any doses of the MMR or MMRV vaccine are considered non-compliant.
0-18 months
No Requirement
Students 18 months old and younger are not required to receive any doses of the MMR or MMRV vaccine.
19 months 1MMR
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 19 months old,
  • the student receives one dose of the MMR or MMRV vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
19 months 1M 1R
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 19 months old,
  • the student receives one dose of the Measles vaccine, and
    • the difference between the second to last dose and the student's birthdate is greater than 12 months with a 4 day tolerance,
  • the student receives one dose the Rubella vaccine, and
    • the difference between the second to last dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

Varicella

Student born before 09/15/1997 are not required to receive any doses of the Varicella or MMRV vaccine.

Students 18 months old and younger are not required to receive any doses of the Varicella or MMRV vaccine.

Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives one dose of the Varicella or MMRV vaccine,
  • the difference between the last dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the student is born between 09/15/1997 and 09/15/2003.
1 dose
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: PK,
  • the student receives one dose of the Varicella or MMRV vaccine, and 
  • the difference between the last dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
2 doses
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives two doses of the Varicella or MMRV vaccine, 
  • the difference between the second to last dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the last dose and previous dose is greater than 28 days with a 4 day tolerance.

Pneumococcal

No Requirement
Students 60 months old and older are not required to receive any doses of the Pneumococcal PCV or PPV vaccine.
0-3 months
No Requirements
Students 3 months old and younger are not required to receive any doses of the Pneumococcal PCV or PPV vaccine.
4-5 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 4 months, but no older than 5 months old, and
  • the student receives one dose of the Pneumococcal PCV or PPV vaccine.
6-11 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 6 months, but no older than 11 months old, and
  • the student receives two doses of the Pneumococcal PCV or PPV vaccine.
19-23 months 4 doses
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 19 months, but no older than 23 months old, and
  • the student receives four doses of the Pneumococcal PCV or PPV vaccine.
19-23 months 3 doses
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 19 months, but no older than 23 months old, 
  • the student receives three doses of the Pneumococcal PCV or PPV vaccine, and
  • the difference between the first dose and the student's birthdate is less than 11 months with a 0 day tolerance.
19-23 months 2 doses
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 19 months, but no older than 23 months old, 
  • the student receives two doses of the Pneumococcal PCV or PPV vaccine, and
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
24-59 months 3 doses
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 24 months, but no older than 59 months old, 
  • the student receives three doses of the Pneumococcal PCV or PPV vaccine, 
  • the difference between the second dose and the student's birthdate is less than 11 months with a 0 day tolerance, and
  • the difference between the last dose and the student's birthdate is greater than 12 months with a 4 day tolerance.
24-59 months 1 dose
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 24 months, but no older than 59 months old, 
  • the student receives one dose of the Pneumococcal PCV or PPV vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 24 months with a 4 day tolerance.

Polio

Students 216 months old and older are not required to receive any doses of the Polio vaccine.

Students are considered compliant if the following conditions have been met:
  • the student is in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives four doses of the IPV, OPV, or IPV/OPV combined vaccine,
  • the difference between the first dose and the student's birthdate is greater than 43 days with a 4 day tolerance,
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the third and previous dose is greater than 28 days with a 4 day tolerance,
  • the difference between the fourth and previous dose is greater than 6 months with a 4 day tolerance, and 
  • the difference between the fourth dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
3 doses IPV
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives three doses of the IPV or IPV/OPV combined vaccine, and
  • the difference between the third dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
3 doses OPV
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12,
  • the student receives three doses of the OPV or IPV/OPV combined vaccine, and
  • the difference between the third dose and the student's birthdate is greater than 48 months with a 4 day tolerance.

3 doses compliant birthdate before 9/15/03

Students are considered compliant if the following conditions have been met:
  • the student's birthdate is on or before 09/15/2003,
  • the student receives three doses of the IPV, OPV, or IPV/OPV combined vaccine, and
  • the difference between the third dose and the student's birthdate is greater than 48 months with a 4 day tolerance.
K-12 Default
Non-Compliant
Students in state grade: KG, 01, 02, 03, 04, 05, 06, 07, 08, 09, 10, 11, 12, who have not received any doses of the Polio vaccine are considered non-compliant.
0-3 months
No Requirement
Students 3 months old and older are not required to receive any doses of the Polio vaccine.
4-5 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 4 months, but no older than 5 months old, and
  • the student receives one dose of the IPV, OPV, or IPV/OPV combined vaccine.
6-18 months
Compliant
Students are considered compliant if the following conditions have been met:
  • the student is at least 6 months, but no older than 18 months old, and

Meningococcal

Students are considered compliant if the following conditions have been met:

  • the student is born on or after 09/15/2004,
  • the student is in state grade: 07, 08, 09, 10, 11, 12,
  • the student receives one dose of the Meningococcal (MCV4 or MenACWY) vaccine,
  • the difference between the last dose and the student's birthdate is greater than 120 months with a 4 day tolerance.
2 doses
Compliant

Students are considered compliant if the following conditions have been met:

  • the student is born on or after 09/15/1999,
  • the student is in state grade: 12,
  • the student receives two doses of the Meningococcal (MCV4 or MenACWY) vaccine,
  • the difference between the second dose and the student's birthdate is greater than 192 months with a 4 day tolerance, and
  • the difference between the second and previous dose is greater than 56 days with a 4 day tolerance.

Students are considered compliant if the following conditions have been met:

  • the student is born on or after 09/15/1999,
  • the student is at least 192 months old,
  • the student is in state grade: 12,
  • the student receives one dose of the Meningococcal (MCV4 or MenACWY) vaccine, and
