Health vaccines and their associated compliance listings are based on state guidelines. While Infinite Campus maintains standard guidelines for all states, districts have the ability to enter additional compliances. The following information describes vaccine compliance rules as defined by the Arizona State Department of Health.

General Compliance

  • Vaccine doses administered less than or equal to 4 days before the minimum interval or age are counted as valid.
  • Doses administered 5 days before the minimum interval or age are be counted as valid doses and must be repeated as age-appropriate. 

Hepatitis B

The following vaccine dose requirements apply to students who are not yet enrolled in kindergarten or higher.

  • 1 dose required for ages 0-3 months.
  • 2 doses required for ages 4-11 months. The difference between the second dose and the first valid dose must be greater than or equal to 28 days and has 4 day tolerance.
  • 3 doses required for ages 12 months and older, with the last does on or after 24 weeks of age.
    • The difference between the second dose and the first valid dose must be greater than or equal to 28 days and has 4 day tolerance.
    • The difference between the last dose and the second valid dose must be greater than or equal to 56 days and has a 4 day tolerance.
    • The difference between the last dose and the first valid dose must be greater than or equal to 168 days and has a 4 day tolerance.
  • 4 doses compliant for ages 12 months and older.
    • The difference between the second dose and the first valid dose must be greater than or equal to 28 days and has 4 day tolerance.
    • The difference between the last dose and the second valid dose must be greater than or equal to 56 days and has a 4 day tolerance.
    • The difference between the last dose and the first valid dose must be greater than or equal to 112 days and has a 4 day tolerance.

The following vaccine dose requirements apply to students who are enrolled in kindergarten or higher.

  • 4 doses required for compliance
    • The difference between the second dose and the first valid dose must be greater than or equal to 28 days and has 4 day tolerance.
    • The difference between the last dose and the second valid dose must be greater than or equal to 56 days and has a 4 day tolerance.
    • The difference between the last dose and the first valid dose must be greater than or equal to 112 days and has a 4 day tolerance.
  • 3 doses required for compliance with the last dose on or after 24 weeks of age.
    • The difference between the second dose and the first valid dose must be greater than or equal to 28 days and has 4 day tolerance.
    • The difference between the last dose and the second valid dose must be greater than or equal to 56 days and has a 4 day tolerance.
    • The difference between the last dose and the first valid dose must be greater than or equal to 168 days and has a 4 day tolerance.
  • 2 doses of Adult Hepatitis B required for compliance.
    • The difference between the first dose and the student's birthdate is greater than 132 months with a 4 day tolerance.
    • The difference between the second dose and the student's birthdate is less than 191 months with a 4 day tolerance.
    • The difference between the second dose and the previous dose is greater than 4 months with a 4 day tolerance.

When a student is not compliant but has met the following requirements, that student in considered to be IN PROCESS.

  • 1st dose within the last 2 months.
  • 2nd dose within the last 4 months.
  • 3rd dose within the last 4 months.

DTaP

4 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, KG: Kindergarten,
  • the student receives 4 doses of the DT, DTap/DTP, Td, or Tdap vaccine, and
  • the difference between the last dose and the student's birthdate is greater than 48 months with a 4 day tolerance.

6 doses under 7 years

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is no older than 83 months old,
  • the student is in state grade: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, KG: Kindergarten, and
  • the student receives 6 doses of the DT, DTap/DTP, Td, or Tdap vaccine.

3 doses 7 years and up

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 84 months (7 years) old,
  • the student is in state grade: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, KG: Kindergarten, 
  • the student receives 3 doses of the DT, DTap/DTP, Td, or Tdap vaccine, and
  • the difference between the first dose and the student's birthdate is greater than 12 months with a 4 day tolerance.

1 doses K-12

In Process

Students are considered in process when the following conditions have been met:

  • the student is in state grade: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, KG: Kindergarten, 
  • the student receives 1 dose of the DT, DTap/DTP, Td, or Tdap vaccine, and
  • the difference between the last dose and the effective date is less than 28 days with a 1 day tolerance.

2 doses K-12

In Process

Students are considered in process when the following conditions have been met:

  • the student is in state grade: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, KG: Kindergarten, 
  • the student receives 2 doses of the DT, DTap/DTP, Td, or Tdap vaccine, and
  • the difference between the last dose and the effective date is less than 5 months with a 5 day tolerance

3 doses K-12

In Process

Students are considered in process when the following conditions have been met:

  • the student is in state grade: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, KG: Kindergarten, 
  • the student receives 3 doses of the DT, DTap/DTP, Td, or Tdap vaccine, and
  • the difference between the last dose and the effective date is less than 5 months with a 5 day tolerance.

Default K-12

Non-compliant

Students in state grade: 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12, KG: Kindergarten, who have not received any doses of the DT, DTap/DTP, Td, or Tdap vaccine are considered non-compliant.

0-1 months

No Requirement

Students 1 month old or less are not required to receive any doses of the DT, DTap/DTP, Td, or Tdap vaccine.

2-3 months

Compliant

Students are considered compliant when the following conditions have been met:

  • student is at least 2 months and no older than 3 months old, and 
  • the student receives 1 dose of the DT, DTap/DTP, Td, or Tdap vaccine.

4-5 months

Compliant

Students are considered compliant when the following conditions have been met:

  • student is at least 4 months and no older than 5 months old, and 
  • the student receives 2 doses of the DT, DTap/DTP, Td, or Tdap vaccine.

6-14 months

Compliant

Students are considered compliant when the following conditions have been met:

  • student is at least 6 months and no older than 14 months old, and 
  • the student receives 3 dose of the DT, DTap/DTP, Td, or Tdap vaccine.

15 months and older

Compliant

Students are considered compliant when the following conditions have been met:

  • student is at least 15 months old, and 
  • the student receives 4 dose of the DT, DTap/DTP, Td, or Tdap vaccine.

Students are considered in process when the following conditions have been met:

  • the student receives 1 dose of the DT, DTap/DTP, Td, or Tdap vaccine, and
  • the difference between the dose and the effective date is less than 28 days with a 1 day tolerance.

  • the student receives 2 dose of the DT, DTap/DTP, Td, or Tdap vaccine, and
  • the difference between the dose and the effective date is less than 5 months with a 1 day tolerance.

  • the student receives 3 doses of the DT, DTap/DTP, Td, or Tdap vaccine, and
  • the difference between the last dose and the effective date is less than 5 months with a 5 day tolerance.

Students are considered compliant when:

  • the student is at least 132 month (11 years) old,
  • the student receives one dose of the Tdap vaccine.

Age 11+ Booster

No Requirement


Start Date: 07/01/2014

Students 132 months (11 years) old and older who receive one dose of DT, Tdap, Td, DTaP are not required to receive a booster when the difference between the last dose and the effective date is less than 59 months, with a 1 day tolerance.

Students 144 months (12 years) old and older are not required to receive any doses of the Tdap vaccine.

Hib

The following vaccine dose requirements apply to students who are not yet enrolled in kindergarten or higher.

  • No requirements for ages 0-1 months
  • 1 dose required for ages 2-3 months
  • 2 doses required for ages 4-5 months
  • 3 doses required for ages 6-14 months.
  • 4 doses required for ages 15 months - 4 years.
  • 1 dose required given on or after 15 months of age.
  • 3 doses require with one dose given on or after 12 months of age
  • No requirements for students 5 years and older.

The following vaccine dose requirements apply to students who are enrolled in kindergarten or higher.

  • No requirements

When a student is not compliant but has met the following requirements, that student is considered to be IN PROCESS.

  • Most recent dose was given within the last 2 months.

MMR

The following vaccine dose requirements apply to students who are not yet enrolled in kindergarten or higher.

  • No requirements for ages 0-11 months.
  • 1 dose required for ages 12 months and older.

The following vaccine dose requirements apply to students who are enrolled in kindergarten or higher.

  • 2 doses of MMR required with first dose on or after 12 months of age and
    • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.
  • 2 doses of Measles, Mumps, or Rubella vaccine with first dose on or after 12 months of age and
    • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.

Students are considered In Progress when the following conditions have been met:

  • the student is in state grade: 1, 10, 2, 3, 4, 5, 6, 7, 8, 9, 11, 12, KG,
  • the student receives at least one dose of the MMR or MMRV vaccine, and
  • the difference between the dose and the effective date is less than 28 days with a 0 (zero) day tolerance.

Varicella

Students 11 months old and younger are not required to receive any doses of the Varicella vaccine.
1 Dose
Compliant
Students are considered compliant when the following conditions have been met:
  • the student receives one dose of the Varicella or MMRV vaccine,
  • the difference between the dose and the student's birthdate is greater than 12 months with a 4 day tolerance, and
  • the difference between the dose and the student's birthdate is less than 155 months with a 4 day tolerance.
2 Doses
Compliant
Students are considered compliant when the following conditions have been met:
  • the student receives two doses of the Varicella or MMRV vaccine,
  • the difference between the last dose and the student's birthdate is greater than 156 months with a 4 day tolerance, and
  • the difference between the second and previous dose is greater than 28 days with a 4 day tolerance.

Students are considered In Process when the following conditions have been met:
  • the student's birthdate is equal to or greater than 156 months
  • the student receives one dose of the Varicella or MMRV vaccine
  • the difference between the last dose and the effective date is less than or equal to 28 days with a 0 day tolerance

Polio

Requirements

0-1 month

No Requirement

Students 1 month old and younger are not required to receive any doses of the Polio vaccine.

2-3 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 2 months, but no older than 3 months old, and
  • the student receives 1 dose of the OPV (End Date: 04/01/2016), IPV, or IPV/OPV Polio vaccine.

4-11 months

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is at least 4 months, but no older than 11 months old, and
  • the student receives 2 doses of the OPV (End Date: 04/01/2016), IPV, or IPV/OPV Polio vaccine.

3 doses Preschool

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade PS: Preschool,
  • the student receives 3 doses of the OPV (End Date: 04/01/2016), IPV, or IPV/OPV Polio vaccine, 
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, 
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the difference between the third dose and the previous is greater than 28 days with a 4 day tolerance.

4 doses before 8/7/09

Compliant

Students are considered compliant when the following conditions have been met:

  • the student receives 4 doses of the OPV (End Date: 04/01/2016), IPV, or IPV/OPV Polio vaccine, 
  • the student receives the last dose before 08/07/2009, 
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance,
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the third dose and the previous is greater than 28 days with a 4 day tolerance, and
  • the difference between the fourth dose and the previous is greater than 28 days with a 4 day tolerance.

3 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student is in state grade: KG: Kindergarten, 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 12,
  • the student receives 3 doses of the OPV (End Date: 04/01/2016), IPV, or IPV/OPV Polio vaccine, 
  • the difference between the third dose and the student's birthdate is greater than 48 months with a 4 day tolerance, and
  • the difference between the third dose and the previous is greater than 6 months with a 4 day tolerance.

4 doses

Compliant

Students are considered compliant when the following conditions have been met:

  • the student receives 4 doses of the OPV (End Date: 04/01/2016), IPV, or IPV/OPV Polio vaccine, 
  • the difference between the first dose and the student's birthdate is greater than 42 days with a 4 day tolerance, 
  • the difference between the second dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the third dose and the previous is greater than 28 days with a 4 day tolerance,
  • the difference between the fourth dose and the previous is greater than 6 months with a 4 day tolerance, and
  • the difference between the fourth dose and the student's birthdate is greater than 48 months with a 4 day tolerance.

Students are considered in process when the following conditions have been met:

  • the student receives 1 dose of the OPV (End Date: 04/01/2016), IPV, or IPV/OPV Polio vaccine, and
  • the difference between the dose and the effective date is less than 28 days with a 1 day tolerance.

Meningococcal

Students are considered compliant when the following conditions have been met:

  • the student is at least 132 months (11 years) of age, and
  • the student receives 1 dose of the Meningococcal (MCV4 or MenACWY) vaccine.

Students are considered non-compliant when the following conditions have been met:

  • the student is at least 144 months (12 years) of age, and
  • the student has NOT received any dose of the Meningococcal (MCV4 or MenACWY) vaccine.

This vaccine is only required in Maricopa County.

  • the student is at least 12 months, but no older than 17 months old, and
  • the student receives one dose of the Hepatitis A vaccine.

Students are considered compliant when:
  • the student is at least 18 months old,
  • the student receives two doses of the Hepatitis A vaccine, and
  • the difference between the second and previous dose is greater than or equal to 6 months with a 4 day tolerance.
